INSPECTION and TEST PLAN

Notes for Completion of Inspection and Test Plan (ITP-KAFD CJ2-S2-NP-DAR-JLL-EL-MV-EQPMT-003_Rev.00)
Definitions of Control Types

1. Inspection - The examination of an item or unit of work for complience against the requirments of a specification, Code or work practice, in general, the inspection will be performed in accordance
with the procedure indentified in the "Procedure" column of the ITP, and measured against the aceptance criteria identified for that activity.

2. Monitor - A control requiring observation of an activity by an organisation not directly responsible for that activity e.g. Supervising Consultant monotoring of fabrication procedures. where the
primary responsibility for inspection of the activity rest with the Contractor.

3. Hold Point - Where this control is identified, the activity must not commence until the relevent party is present, or has notified the Contractor in writing that the activity may proceed. Where the
supervising Consultant's presence a hold point against an activity, it is expected that evidence of this control be retained in the Contractor's test records.

4. Witness Point - A control requiring the supervising consultant's presence at an inspection or test and does not normally apply to the Contractor. The contractor is required to give due
notice in writing to the Supervising Consultant that the activity will takes place. The activity may proceed without the Supervising Consultant's or attendance,
provided due notice has been given. Evidance of this control is required to be retained in the Contractor's test records.

5. Accepted - Generally applies to acceptance of specific Contactor procedures by the Supervising Consulatnt's prior to the procedure being implimented.

6. Review - An assessment or examination of the Contractor's or Sub- Contractor's documentation. These may be in the form of inspection and test records, procedure, etc.

Extent of Control

7. Where 100% control of an activity is not required, the ITP may state the percentage required. For example, Sub- Contractor radiographic inspection of pipe welds would be enterd as 10% .
Percentage control does not relate to Hold, Witness or Accept control.

Contract Specific Inspection and Test Requirements

Agreement is required on the following points at the Contract kick off meeting on the following points:

* Where Supervising Consultant's control are identified, what this actually means with regard to sign off of Contractor inspection and test forms. For example, does "w "

* GMP requirments for form sign-off e.g. pen colour, date format, use of correction fluid, etc..

* What evidance is required on Contractor's and authority for performing control activities. The PM discipline will normally be the person authorised to perform contro activities, such as witness,
review etc. on specific tasks, however an agreement is required between is required between PM and the Contractor on responsibilty for performance of Contractor controls.

* Where PM controls are identified for a Contractor activity, the Contractor is required to ensure there is adequate provision for PM sign off on relevent test forms.
