➔ The Bhopal Gas Tragedy :

Location: Bhopal, Madhya Pradesh (3 dec 1984)

Incident: A gas leak from the Union Carbide pesticide

➔ The Kolkata Bridge Collapse (2016) :

Location: Kolkata, West Bengal

Incident: A flyover under construction collapsed,


causing vehicles and pedestrians to fall.

Casualties: 26 deaths and many injuries.

Failures:

Construction Oversight: Poor construction


practices and inadequate quality control during the
project.
Design Flaws: Structural weaknesses due to design
errors were identified.
➔ The Nestlé Maggi noodles controversy :
Nestlé Maggi noodles were involved in a controversy in India in 2015 after
the Food Safety and Standards Authority of India (FSSAI) found excess
lead and monosodium glutamate (MSG) in samples

Background

Maggi is tested at the Central Food Laboratory in Kolkata revealed lead


levels that were over 1,000 times higher than Nestlé India's claims.

Response
Nestlé India temporarily stopped selling Maggi noodles in June 2015. The FSSAI
ordered a nationwide recall of Maggi noodles on June 5, 2015. Nestlé destroyed
over 38,000 tonnes of Maggi noodles and lots of tests it return in nov. 2015 .

Failures:
• Inadequate quality checks and compliance with safety standards.
• Misleading labeling and marketing practices regarding ingredients .
➔ Air India Express Flight IX-1344 :
Location: Kozhikode International Airport, Kerala

Incident: The flight overshot the runway during


landing in heavy rain and broke into two pieces.

Casualties: 21 fatalities, including both pilots, and


many injuries.

Failures:

•Runway Safety: The runway was known for its


challenging conditions, and safety measures were
inadequate.

•Pilot Decision-Making: Issues were raised about


the crew's handling of the landing in adverse weather.
➔ The Kanchanjunga Express Collision :
Happens in June 17, 2024

Location: Malda, West Bengal

Incident: The Kanchanjunga Express collided


with a stationary goods train.

Casualties: Over 50 fatalities and many injuries.

Failures: Signal malfunction and inadequate


maintenance of railway infrastructure were
identified as contributing factors.
