REMARKS
An audiometric test shall :
1. Be conducted by an audiometric technician
2. Be preceded by a period of quiet of at least fourteen hours
3. Be carried out in a room with background noise level meeting the requirements specified in
the Second Schedule
4. Be of pure tone, air conduction, with test frequencies including 500, 1000, 2000, 3000, 4000,
6000 & 8000 Hz taken separately for each.
METHODOLOGY :
1. A mobile unit will be arranged to conduct hearing test at site. Employees required to rest
for 14 hours before carrying out the test
2. Occupational Health Doctor registered with DOSH will review the audiometric testing result
and prepare report
Terms of payment is 30 Days from Invoice date.
Kindly raise the Purchase Order as soon as possible.
