Excerpt from the minutes of the Bids and Awards Committee meeting held on NOVEMBER 14,
2012 at the Barangay Session Hall at exactly 9:30 in the morning.

A RESOLUTION DECLARING THE LOWEST CALCULATED AND RESPONSIVE PRICE


QUOTATION AND RECOMMENDING ITS AWARD OF CONTRACT AND APPROVAL
TO THE PUNONG BARANGAY, HON. PETRONILO E. DEJARME FOR THE GOODS
UNDERTAKEN THROUGH THE ALTERNATIVE MODE OF PROCUREMENT THROUGH
SECTION 53.9 OF RA 9184 OR NEGOTIATED PROCUREMENT - SMALL VALUE
FOR THE USE OF OPERATION OF THE BARANGAY.

WHEREAS, the Barangay of Magwawa wanted to purchase various office supplies charged to
5% GAD Fund of BY 2023 Approved Annual Budget for the operationalization of the
VAWC Desk.

WHEREAS, the Approved Budget for the Contract (ABC) of the said procurement is P48,000.00
and will be procured pursuant to Section 53.9 – Negotiated Procurement – Small Value of 2016
Revised Implementing Rules and Regulations (IRR) of Republic Act (RA) No. 9184 as indicated
in the Approved Annual Procurement Plan for FY 2023.

WHEREAS, the Bids and Awards Committee, through the Secretariat, initiated the procurement
activity by sending the Request for Quotation (RFQ) to three (3) known eligible prospective
suppliers, the hereunder total price quotations were submitted by the following suppliers, to wit:

JPMF ENTERPRISES P 47,500.00
KST MULTIPURPOSE COOPERATIVE P 45,000.00
CBJ CALIBRATION ENTERPRISES P 49,500.00

WHEREAS, after an evaluation of the quotations received from the above-mentioned suppliers,
the same was found compliant and responsive to the immediate need of the end-user;

WHEREAS, on motion by Kagawad Edgar M. Reyes, seconded unanimously, to award the


contract to the known responsive supplier as the single calculated and responsive price quotation.
NOW, THEREFORE, We, the member of the Bids and Awards Committee, hereby RESOLVE
as it is hereby RESOLVED:

To declare KST MULTIPURPOSE COOPERATIVE as the supplier with the Lowest


Calculated and Responsive Price Quotations and recommend for approval to the Head of the
Procuring Entity, Hon. Petronilo E. Dejarme for awarding of contract to the aforementioned
supplier amounting to P45,000.00 for procurement of supplies.

RESOLVE, this 29th of January 2023 at the Barangay Session Hall, Brgy Magwawa, Santo
Tomas, Davao del Norte,
