Registration Instructions For Chip Design Implementation Course
一、報名資格
(一)學員報名資格請參各梯次課程簡章公告。
(二)此系列課程因課程含晶片實作、下線製作之課程內容,故各課程報名學員資格本中
心會嚴格審核,凡經查核發現報名不符合資格者,一律不給予上課,亦不得要求退費
或更換上課人員,故請欲報名學員於報名前務必確認是否符合報名資格。
1. Registration Eligibility
(1) Please refer to the course prospectus for each session for participant eligibility.
(2) Due to the practical chip design and implementation content of this course series, participant
eligibility for each course will be rigorously reviewed by TSRI. Those found to have
registered without meeting the eligibility criteria will not be allowed to attend the course.
Refunds or replacement of attendees will not be granted. Therefore, prospective participants
are kindly requested to confirm their eligibility before registering.
二、報名方式
一律採網路報名,恕不接受其他報名方式。
2. Registration Method
All registrations must be completed online. Other methods of registration will not be accepted.
三、課程相關注意事項
(一)本課程僅開放給本國籍學界學員報名,且不接受代理上課,欲上課者請務必以上
課本人名義進行報名,若非報名本人一律不給予上課,亦不得要求退費。
(二)學員每日上課簽到/退請務必持報名本人之證件簽到/退,若上課當天未攜帶證件者
恕無法辦理報到及上課。
(三)此系列部份課程因專題實作而需使用製程資料或元件庫資料,若為此類課程於上課
前每位學員皆需完成「製程資料授權使用切結書」或「製程及元件庫資料授權使用切
結書」簽署,經本中心審核資料無誤後,才可正式上課,凡未簽署或經查核資料不符
者,依規定一律不給予上課,亦不得要求退費。
(四)每日上課前及課程結束皆需簽到及簽退,課後恕不接受補簽到/退。
(五)本系列課程考量課程專題實作連續性,故課程上課每位學員教室座位係為固定座位,
因此請學員每日上課依本中心指定的座位入座上課。(座位表於上課當天會公告於上
課教室)
(六)於課程開課日前一星期報名之學員,上課當天請攜帶繳費單收據至上課處報到。
(七)本系列課程恕不提供單購講義。
(八)證書發放及免費晶片製作規定:
出席率(以簽到退表計算)達上課總時數 80%(含)以上,即會發予「結業證書」。
完成晶片製作並通過下線規格者,可進行晶片製作,並發予「晶片實作證書」。
3. Course-related Notices
(1) This course is only open to participants of the same nationality and affiliated with the
academic community. Proxy attendance is not allowed. Participants must register under
their own names to attend the course. Those who have not registered under their own name
will not be allowed to attend, and refunds will not be granted.
(2) For daily attendance, participants must sign in/sign out with their own identification.
Those without proper identification on the day of class will not be allowed to attend.
(3) For some courses in this series that involve special projects, participants may need to sign a
“Process Data Authorization Affidavit” or “Affidavit for Authorization of Use of Process
and Cell Library Data” before the course. After TSRI verifies the accuracy of the submitted
data, participants can proceed to attend the course. Those who have not signed the affidavit
or whose data doesn't match the requirements will not be allowed to attend, and refunds
will not be granted.
(4) Signing in/signing out is required before and after each class. Late sign-ins/sign-outs will
not be accepted.
(5) Due to the continuity of course projects, seating in the classroom is assigned and fixed for
each student. Therefore, participants are requested to sit in the designated seats as instructed
by the center for each class. (Seating arrangements will be announced in the classroom on
the day of the class.)
(6) For participants who registered one week before the course starts, please bring the payment
receipt on the day of the class for registration.
(7) Single purchase of handouts is not available for this course series.
(8) Certificate Issuance and Free Chip Implementation Regulations:
If attendance rate (calculated from sign-in/sign-out records) reaches 80% or higher of
the total class hours, a “Certificate of Completion” will be issued.
Participants who complete chip implementation and meet specified criteria will be
eligible for free chip implementation and receive a “Chip Implementation Certificate.”
四、退費及調課機制
(一)課程更換
本系列課程恕不接受調課或更換課程,報名後若因故無法上課者,一律以退費處理,
課程退費申請詳參課程退費規定。
(二)課程退費
1.欲申請退費者,請於各課程報名截止日前備齊退費所需文件向承辦人員提出退
費申請,凡逾期或未備齊所需文件,恕不受理。
2.申請退費者,需酌收 1/4 報名費為行政處理費,,其餘餘款則一律以匯款方式退
回申請人指定帳戶(需為申請人本人之帳號)。
3. 退費申請所需之文件:
(1) 課程退費申請表(請至訓練課程網頁/相關表單下載)
(2) 便利商店已繳費收據影本
(3) 欲退款之存摺封面影本(含帳號、戶名、開戶行等資料以供判讀)
4. 退費申請文件,需於該課程報名截止日前 Email 承辦人員。
(於文件送出請務必與承辦人員確認,以免因逾期而造成無法退費)

五、其他注意事項
(一)本中心課程一律嚴禁旁聽、錄影、錄音及拍照,敬請學員配合,若經發現屆時將
會轉知您的指導教授/公司,並將影響您未來至本中心上課之權利。
(二)課程期間如遇天災(颱風、地震…等因素)停課,停課標準以上課所在地縣市政府宣
佈為準。若停課則考量課程延續性問題,課程會順延至假日(週六或週日)補課,相
關補課事宜會再另行通知及公告。
(三)午餐請自理,本中心恕不提供代訂便當之服務。
(四)中心保留講師、上課時間、教室更換之權利。
(五)於課程期間,若有任何問題,請各位學員隨時與課程工作人員反應(若有時效性之
問題者,請務必及時反應),以免影響您的權利或造成您的不便。
(六)其他相關規定詳參各課程簡章或本中心訓練課程網頁公告。
5. Other Notices
(1) TSRI strictly prohibits any form of auditing, recording, audio recording, or
photography during the courses. We kindly ask for your cooperation. If any violations
are detected, your supervising professor/company will be informed, and it may impact
your future attendance at TSRI.
(2) In case of natural disasters (such as typhoons, earthquakes, etc.) during the course period,
class suspension will follow the announcements of local county/city governments where the
classes are held. If classes are suspended, considering the continuity of the course, makeup
classes will be scheduled on weekends (Saturdays or Sundays). Details regarding makeup
classes will be communicated and announced separately.
(3) Lunch is not provided by TSRI. Participants need to make their own arrangements and we
do not offer lunch box catering services.
(4) TSRI reserves the right to change instructors, class times, and classrooms.
(5) If you encounter any issues during the course, please feel free to communicate with the
course staff at any time (especially for time-sensitive matters, prompt communication is
necessary) to avoid any inconvenience or impact on your rights.
