Confirmation of the certification request

By signing this certification request we confirm that all information provided are correct. Furthermore we confirm that in case
our company represents a conversion unit (last interface) that we will provide the certification body with a copy of each Proof
or Partial Proof of Sustainability within 24 hours after issuing , if this documents are issued for deliveries to Germany. We
herewith confirm that the contact person 1 has the right to receive the audit report(s). It should be noted that all powers
conferred by a closed contract, set up between the standart owners concerned and the accreditation body and Cotecna as
certification body, are also part of the later contract (e.g. the standard owner and/or accreditation body may at any time
accompany the audits. This can be witnessaudits or parallel audits but the results of the audit may be reviewed by those
named 3rd [parties, these need to be informed and involved also during complaints proceedings). In addition, the customer
who enters into a contract with Cotecna certification body has the right to appeal. The potential complaint is then proceed in
accordance with established procedures of Cotecna certification body and the contractual procedures regarding the
certification and the standard owner and/or the accreditation body. A contract becomes binding when a client accepts our
respective offer
