1. PURPOSE
The purpose of this plan is to establish standardized sequence or criteria
through which authorization for use of vehicles or trucks will be strictly adhered
to throughout the execution of the Pipeline Rehabilitation and Repair Scope of work.
2. SCOPE
This plan applies to all PPSL personnel who will transit from one location to
another for related purposes to the Project. It explains sequence which through
safe procedures shall be applied towards achieving cohesive movement of
personnel and equipment either for onshore or offshore purposes.
3. ROLES AND RESPONSIBILITIES:
3.1 Project Manager (PM):
The PM is responsible for ensuring that all requirements specified in the
approved procedures are performed accordingly within the AWO requirements
for the project. The PM is the communication route through which Pipeline
Rehabilitation and Repair project expectations are transmitted to the PMT
designated for this project.
3.2 Human Resources (HR):
The HR is responsible for all PPSL personnel directly and indirectly involved in
the execution of project. The responsibilities of the HRM include the following
but not limited to;
Approval of movement of personnel
Ensures all availability of fuel products for movement
Ensures appointed personnel attend the relevant training programs
Maintains a record and tracking system for all personnel
3.3 Logistics Coordinator (LC):

The logistics coordinator is responsible for the coordination of journey plans for
all personnel to be transited from one location to another. The responsibilities of
the LC include the following but not limited.
Assigning of Driver for journeys
Receipt of all freight waybills both incoming and outgoing
Custodian of all vehicle particulars and ensuring their validity is renewed a
month to its expiry date
Maintains a register of all equipment supplied in or sent out.
3.4 Safety Officer:
It is the responsibility of the safety officer to ensure that all safety measures are
in place and strictly adhered to. The safety officer will be responsible for safety
of all personnel onshore and offshore. The responsibilities of the safety officer
include but not limited.
Coordination of all SHE activity
Ensure that all personnel designated to travel are medically fit
Ensure drivers and operation personnel perform and pass the drug and
alcohol test prior to any activity
Ensure that all vehicles, trucks or zodiacs are appropriately equipped with
safety kits (first aid packs).
3.5 Truck Driver:
The truck driver is responsible for the transportation of materials within onshore
locations. The truck driver ensures that all goods or equipment carried by his
designated truck is fastened appropriately prior to transportation.
3.6 Vehicle Driver:
The vehicle driver is responsible for the transportation of personnel within
onshore location. He takes on the full safety responsibility of all personnel once
in active working conditions (Driving on the roads). He ensures that all personnel
under his charge implement approved safety rules and regulations to his
vehicles. Example: Always wear your seat belt once you enter the vehicle.

3.7 SPDC Representative:
SPDC representative is responsible for ensuring that smooth and coherent
running of operations between PPSL and SPDC.

4. GENERAL RULES
For all remote trips, the relevant PPSL logistics coordinator shall ensure the
driver is authorized and register the following:
Driver name and number of passengers
Vehicle number
Departure time
Destination
Drive Right monitoring device operational (Y / N)
It is the logistics coordinators full responsibility to ensure that the information
provided by the driver on journey request checklist form is correct. The
supervisor is expected to carry out random spot checks on the vehicle(s) prior to
departure. Night driving is not allowed, as per PPSL policy, unless deemed very
necessary; in which case, a formal permission shall be granted by the Project
Manager with the authorization of the HRM. It is the Project Managers
responsibility to liaise with client representative prior to authorizing any night
driving. Off road driving at night is completely forbidden. However, off road
driving during daylight may be allowed only for certain circumstances and only if
supervisor permission is granted beforehand. It is the driver responsibility to
ensure that the vehicle is properly maintained and that it is in good condition
prior to departure. He must check the following as minimum.
Tyres including spare tyre
First aid tool box availability
Fluorescent triangle availability
Engine, battery, cooling/heating system, fuel and oil
Functional working brakes and warning signs / alarms
Available functional fire extinguisher
Seat belts are operational

5. JOURNEY RULES
It is the full responsibility of driver to adhere to PPSL, Client, and local authority
driving regulations. The most stringent regulations are always enforced. These
regulations include but not limited to the following:
a. Driving is allowed only with an authorized vehicle permission, the driver
valid license shall always be with him during a trip.
b. Vehicle must be checked prior to departure. Journey check list form to be
filled and signed by the applicable principal officers.
c. No driving under the effect of alcohol or drugs.
d. Do not start journey if you feel sick or tired.
e. Ensure fire-fighting extinguisher and first aid kit are available and are
functional.
f. Ensure all desert equipment (such as spare wheel, jack, drinking water,
fuel, etc. are available and in good functional conditions.
g. Secure all long loads protruding from the rear of a vehicle shall be clearly
marked with approved sign recognizable by all agencies.
h. Before starting the vehicle, ensure the area near, behind, and under
vehicle is human free.
i. Ensure safety belts are used properly (driver & passengers). Number of
available and operational seat belts limits the number of passengers. No
passengers are allowed to ride on the sides or on the back of any vehicle.
j. Ensure the Drive Right monitoring device is connected and operational.
k. Smoking in vehicles is prohibited.
l. Record the journey at PPSL yard gate.
m. On the road, the head lights shall always be on.
n. All relevant road speed limits must be complied with.
o. Always confirm arrival to job location, if possible (use available radio /
telephone).
p. Minimize driving backward to necessity.
q. If parking on the road is necessary; vehicle shall be parked only in
allowable places, alongside the road, far from bends and road crossings.

r. The Vehicle must be visible from at least 40m and markers shall be used
if necessary. Completely parked vehicles shall have the ignition switched
off and all doors locked. All drivers must apply the “Defensive Driving
Rules “e.g. consider and avoid the mistakes of others. To complete the
cycle and ensure safety, PPSL will follow the vehicle manufacturer
maintenance instructions and procedures.
6. NIGHT DRIVING PROCEDURE
When driving at night is important to rescue company personnel or for some
unforeseen operational reasons, the following procedure should be adhered to:
a. Project Manager shall approve the journey and liaise with client
representative to Issue required permit if applicable.
b. A night authorized driver who is allowed for night driving should only be
selected.
c. The normal driving journey check list should be filled out with the following
additions to cover driving at night requirements: lights, availability of
adequate fuel, food and water and the full details of the rout, torch…etc.
Human Resource Manager should nominate a duty contact person in the
base.
d. Driver should inform the duty contact person upon arrival to destination.
e. Unless there is necessity to drive back (i.e., only in the case of severe
injury), driving back should not be permitted.
f. A copy of this procedure should be distributed to all drivers.

7. TRIP MANAGEMENT
Logistics coordinator will ensure that all journey management plan policies is
established and necessary travel request is submitted for approval. Logistics
coordinator shall ensure that trip management focus on;
Implementation of a journey management plan policy
Method of transportation that matters for appropriate approval.
Ability to reduce friction and frustration over expenses and reimbursement
Review the status of all trips.
a. Determining Necessity Of The Trip
Logistics coordinator will ensure that travel within organizations can often
be minimized by combining trips.
Ensure travel request is submitted timely with accurate supporting trip
documentation.
Approving payment of the travel claim in a timely manner.
If commuting is required, the alertness of the driver and possible alternatives to
driving such as car-pooling, public transportation or taxi services should be
considered.
b. Assessing Trip Hazard
Logistics department conduct a trip hazard assessment to identify health
and safety hazards and evaluate the risks presented within the
equipment, cargo and the surrounding environment.
Evaluate the risk and decide whether the existing control measures are
adequate or whether more should be done.
A hazard register may be used to capture specific details of the hazards,
as well as the corresponding controls. This data will be kept for up to two
years after the end of the project.

Logistics department ensure additional controls are implemented
wherever necessary and to prioritize further resources if needed to ensure
above.
c. Registering Risk On A Referenced Risk Register
The PPSL risk register would document additional risks for relevant routes.
Previous incident reports and driver experiences are included with the hazards
identified for different routes in a risk register. The consolidated information can
be used by the logistics department to improve the risk assessment for future
use.
d. Risk Controls
Logistics department will develop controls that are appropriate for each
risk exposure previously identified. PPSL will ensure that Risk controls
focuses on;
Establishing a dedicated risk management team responsible for
monitoring and responding to emerging risk.
Encourage a culture of open communication and collaboration, enabling
drivers to share insight and concerns about potential risk.
Evaluating risk and establishing suitable precautions.
Reviewing your assessment and re-assessing if necessary.
PPSL will ensure that risk control consider regulatory compliance and
industry best practice.
e. Trip Planning
PPSL logistics department will ensure that prior to any trip they will embark
upon, it is important to plan all the way from harnessing available resources,
proper allocation and efficient utilization of available resources, towards
completing the trip safely and on a timely manner. The process or proper trip
planning involves:
Planning a successful trip and prioritize keeping the personnel/company
staff as productive and stress-free as possible.
This requires carefully considered, proper planning and a detailed
itinerary.
Organize proper trip planning as possible beforehand against unforeseen
circumstances.

Confirm funding is available before proceeding to the point of destination.
f. Evaluating The Trip Plan
Assessing the quality of the trip plan documents and the implementation of the
trip plan action to understand what interventions were and were not enacted in
workplaces.
Appraising the level of organizational commitment to initiative, including
management engagement, allocation of resources and links to strategic goals.
In an effort to continually improve and update trip plans, PPSL will ensure that
driver can check and provide input to trip plans.
The plan may require approval and signatures of the driver and an appropriate
level of supervisor or Logistics coordinator.
g. Preparing For the Trip
Once the trip has been planned and approved, preparations can be made for the
trip. Logistics will ensure that the vehicle is reliable, well maintained, and
designed for the planned journey. PPSL logistics coordinator or the chief driver
will inspect the vehicles according to the company’s standard, OR a higher
standard where applicable.
Logistics department will ensure that measurement of outcome is allocated,
and establishing management commitment early in the process toward
achieving the goal.
As operating conditions can and do change constantly, PPSL will ensure that
the trip plan is revisited during the preparation stage to respond to changes in
road conditions, community conflict, weather, visibility, traffic, and the vehicle.
Logistics department will ensure that cargo is secure, weights and dimensions
are compliant, seasonal restrictions are well anticipated ahead of time, (road
bans, restricted travel around holidays, etc.), required maintenance and
inspections have been completed, also important to confirm the driver’s fitness
for duty or fatigue level that may result in reduced alertness.
h. Undertaking the Trip
PPSL shall ensure that a well-planned and prepared trip with all appropriate
approvals secured is achieved so as to help the driver concentrate on the road
with confidence.
The driver should notify the appropriate superior when the trip has commenced.

While the trip is executed, PPSL shall ensure that necessary action are taken to
ensure the driver, vehicle and personnel are safe and secured against any claim
or community conflict.
Periodic rest stops, scheduled stops to inspect cargo and equipment, periodic
communication would occur with the Logistics coordinator at pre-determined and
agreed to intervals and also to ensure that there is monitoring.
PPSL will ensure that the have the correct documentation and perform the
required business activities, prior to the trip departure.
i. Completing the Trip
PPSL logistics coordinator shall ensure that a post-trip inspection and check-in
is carried out, to debrief the trip and close out the journey.
The PPSL logistics coordinator at the completion of the trip, would require of the
driver to report any near misses so that improvements can be made to the trip
plan.
The PPSL logistics coordinator shall ensure that a Post-trip vehicle inspections
that can identify necessary repairs and ensure the vehicle is in safe condition for
the next trip, is being carried out.
Logistics coordinator shall review and approves the expense details and
processes transaction for final review and payment for the trip.

10.TRAINING
PPSL shall ensure that a debrief process is regularly done to afford an
opportunity to report and share information about the trip. This will continuously
refine and improve the hazard identification and risk assessment process.
PPSL will ensure that drivers attend lessons learned session on a quarterly
basis so as to discuss and learn newer and more efficient ways of discharging
their duties, PPSL will not just focus on driving skills and vehicle control, but also
on higher level issues such as journey planning and how personal
characteristics can influence risks. The aim of this training is to help drivers
recognize how they can minimize their own risks. It is likely to be more effective
if it includes opportunities for interaction and group discussion so that staff can
share their experiences and express their views and new information about
hazards or updates to the conditions prior to improve the safety for the next
driver and / or subsequent trips.
