JOINT AFFIDAVIT
(As required in Article 34 of the New Family Code)

WE, GARRY BELARA and LUCIA B. LENARES, of legal age, Filipino citizen, single,
resident of and with postal address at Panaguikan, Tapon, Dalaguete, Cebu after having been duly
sworn to in accordance with law, do hereby depose and say:

1. That we have been living together as husband and wife for the past Thirteen (13)
years without the benefit of marriage and blest with six (6) children namely Ma.
Israelita B. Lenares, Loren Gayle Lenares, Von Saymon Lenares, Anthon June B.
Lenares, Abelle Athrio B. Lenares and Luigi Timothy B. Lenares per Certificate of
Birth and Baptism hereto attached;

2. That we have all the qualifications to contract marriage;

3. That we would like to legitimize our relationship by contracting marriage legally and
avail of the provision of Article 34 of the new Family Code;

4. That we are executing this affidavit to support the facts above-mentioned and
establish our intention to be married legally;

IN WITNESS HEREOF, I have hereunto affix our signatures this 21st day of October,
2003 at Dalaguete, Cebu, Philippines.
