INTRODUCTION: The researchers are currently conducting research entitled "The Satisfaction
of Employees and Customers on Self-Service Kiosk in the Selected Fast Food Restaurant in
the City of Malolos." This study aims to know the satisfaction of both employees and customers
on the use of a self-service kiosk in a fast food restaurant. In terms of the customer's order accuracy,
convenience, ordering speed, and menu design.
SECTION A: The researchers will be considering the demographic profile of the respondents.
DIRECTION: In order to answer the survey questionnaires, respondents need to fill in the
following: age, sex, highest educational attainment, employment status, and frequency of visit(s)
to a fast food restaurant with a self-service kiosk.

SECTION B: In this section, it will identify the customer's satisfaction in terms of ordering speed,
order accuracy, convenience, and menu design.
DIRECTION: In order to answer the survey questionnaires, put a check (/) on the designated raw
from (1) Strongly Agree to (5) Strongly Disagree. Researchers will guarantee the privacy and
safety of the respondents regarding the answers from the survey questionnaire. The results of the
questionnaires that came from the respondents will only be used for the intended study.
