Community Health Centre

PATIENT You are 35 years old and exercise every day. You are worried about being
underweight. You would like some advice.

• When asked, say you’ve been on an intensive exercise programme at the local
gym for three months and you exercise a few hours a day every day, but you’re
worried about losing too much weight, so you’d like some advice on what to eat
so that you don’t need to give up doing exercise.
• When asked, say you just exercise because you really enjoy it and it’s one of the
main ways you socialise. When asked, say you now weigh 60 kg (132 lbs).
You’re 165 cm (5 ft 4 in) tall. You’ve lost 10 kg (22 lbs) over the last 12 months,
so your BMI (body mass index) is now 22.
• When asked, say you think you eat pretty healthily; you try to eat different food
groups, for example, you’ll often have meat with vegetables or salad for dinner.
You eat a small lunch and dinner but you often skip breakfast. You sometimes
have fruit or an energy bar before exercising as a snack.
• When asked, say you know there are some foods you can eat that will help you
put on weight, like high protein egg drinks and eating lots of meat, but you’re
worried about having high cholesterol because your father has high cholesterol.
• Say that advice has been really useful; you’ll make those changes.

DIETITIAN You see a 35-year-old patient who exercises every day and is worried about being
underweight. He/she would like some advice.

• Find out reason for patient’s visit.


• Explore reasons for exercise regime (maintaining weight, job, lifestyle choice,
enjoyment, etc.). Find out relevant patient details (weight, height, 12-month
weight history, etc.).
• Advise need for correct diet before exercising (e.g., providing fuel/energy,
preventing lethargy/dizziness, higher calorie intake than calories burned, etc.).
Find out details about patient’s diet (healthy and balanced, regularity and size of
meals, any snacking, etc.).
• Emphasise importance of breakfast (e.g., replenishing blood sugar, providing
energy, preventing hunger/binge eating, etc.). Make dietary recommendations
(e.g., complex carbohydrates: whole grains, fruits, vegetables; lean proteins:
poultry, fish, dairy; unsaturated fats: nuts, seeds, avocados; etc.). Find out any
other questions/concerns.
• Reassure patient about high cholesterol (e.g., reduced risk with regular exercise,
preventable with correct dietary choices, etc.). Advise on prevention of high
cholesterol (e.g., avoiding saturated and trans fats: fatty meat, full-fat dairy,
packaged snacks/desserts; limiting salt and alcohol intake; etc.).
