INSTRUCTIONS

This ‘Consent Form’ (‘Form’) has been developed to provide you with important information regarding your visa application. This
Form includes a Privacy Statement, Information required by the Code of Conduct for registered migration agents and other
important information.

This Form accompanies the ‘Client Information Questionnaire’ (‘CIQ’) (also included in your Commencement Email), which is used
to collect information to enable KPMG’s registered migration agents to complete and lodge a visa application on behalf of you and
any accompanying secondary visa applicants with the Australian Department of Immigration and Border Protection.

You should read this Form before you complete the CIQ.

IMPORTANT:
Please note your KPMG registered migration agent will prepare your visa application based on information that you provide in the
CIQ.
Please ensure that you provide complete and accurate responses to each question.

Instructions for completing this Form:


You must:
• Read Sections 1 (Privacy Statement), 2 (Information required by the Code) and 3 (Other Important Information) of this Form.
• Make the contents of the Privacy Statement in Section 1 known to each individual whose personal information will be provided
in the CIQ.
• Either:
o Complete this Form online:
• Read the declarations set out in Section 4 (Declarations) of this Form
• Confirm your agreement to each of the declarations by selecting ‘yes’ (if applicable). Note: when completing the
declarations, please review the declarations in conjunction with the Privacy Statement set out in Section 1.
• To provide your consent, type your name and the date in Section 4.3 (Declarations) of the Form in the space
provided.
• Save and email the Form to your KPMG immigration contact; OR
o Complete this Form by hand:
• Print this Form and read the declarations set out in Section 4 (Declarations) of this Form
• Confirm your agreement to each of the declarations by ticking ‘yes’ (if applicable). Note: when completing the
declarations, please review the declarations in conjunction with the Privacy Statement set out in Section 1.
• To provide your consent, sign and date the declaration in Section 4.3 in the space provided.
• Scan and email the Form to your KPMG immigration contact.

PLEASE NOTE KPMG MUST RECEIVE YOUR COMPLETED CIQ AND THE SIGNED DECLARATIONS CONTAINED IN THIS
FORM IN ORDER TO PREPARE YOUR VISA APPLICATION.

SECTION 1: PRIVACY STATEMENT

KPMG collects personal information, including sensitive information, as part of the immigration services provided to you by its
registered migration agents (for example, through the CIQ). KPMG will treat the information in accordance with the Australian
Privacy Principles established by the Privacy Act 1988 (Cth).
This statement summarises how KPMG collects, uses, discloses and manages personal information collected as part of the
immigration services it provides to its clients and their employees and family members through its registered migration agents.
This statement should be read in conjunction with KPMG’s Privacy Policy (accessible by clicking on this link).

Collection of personal information

As part of providing immigration services to you, KPMG (through its registered migration agents) may collect personal information
from:
• you directly at any stage of the visa application process
• your spouse, partner or dependents
• your employer
• any party whom you authorise to provide us with such information

Purpose, use and disclosure of personal information

KPMG will use your personal information (including unless you tell us otherwise, sensitive information) to:
• provide assistance to you in relation to your application for a visa
• consider and lodge your visa application on your behalf as your registered migration agent
• confirm your visa status using Visa Entitlement Verification Online (VEVO)

KPMG’s registered migration agents are bound by the Office of the Migration Agents Registration Authority’s ‘Code of Conduct for
Registered Migration Agents’ (‘Code’). As required under the Code, we will only disclose confidential personal information about
you with your written consent.

Subject to our obligations under the Code, the parties KPMG may disclose your personal information to include:
• your employer and its associated entities
• your spouse or partner
• the Department of Immigration and Border Protection
• foreign authorities or regulators relevant or applicable for the purposes of providing immigration services to you
• third party agencies who process and approve foreign visa applications on behalf of overseas governments
• government agencies for the purpose of assessing positions in regional areas
• institutions and professional organisations in order to assess your qualifications and memberships
• translators
• personnel within KPMG for the purposes of them providing tax and mobility services to you (e.g. lodging tax returns on your
behalf whilst you are working in Australia)
• other KPMG member firms
• KPMG’s external service providers on a confidential basis.

What happens if you do not provide your personal information

If you do not provide the personal information requested, then we may not be able to:
• provide you with the services we were engaged to provide including completing and lodging your visa application;
• make the complete disclosures that are necessary to lodge your visa application; or
• provide advice that is complete and accurate.

Disclosure of personal information overseas

We may send some of your personal information outside of Australia if:


• your employer is outside of Australia;
• the third parties listed above are located overseas; or
• we are providing immigration services in a foreign country.

Personal information you provide in relation to your visa application will be stored electronically on databases with restricted
access. The databases on which your personal information may be stored are located in the United States of America and
Singapore.

Access to your personal information and complaints

The KPMG Privacy Policy contains information about how you can:
• access and seek correction of your personal information; and
• how you may complain about a breach of Australian Privacy Principles.

SECTION 2: INFORMATION REQUIRED BY THE CODE


2.1 GENERAL
Registered migration agents are bound by the Office of the Migration Agents Registration Authority’s (‘MARA’) Code of Conduct.
The Code is intended to regulate the conduct of registered migration agents and provides protection to our clients. KPMG will
provide you with a copy of the Code on request.
Under the Code, we are required to provide you with a copy of MARA’s Consumer Guide. The Consumer Guide provides
information about the migration advice profession, the functions of MARA, the legislation regulating the profession, what a client
can reasonably expect from a registered migration agent and information about the complaint procedures. A copy of the
Consumer Guide has also been provided to you as an attachment to your Commencement Email. We encourage you to read it.
The Code stipulates that clients may request a copy of their application and any related documents. KPMG may at its discretion
charge a service fee in order to provide a copy of your file.

2.2 CONFLICTS OF INTEREST

Under this engagement, KPMG’s registered migration agents will act as your registered migration agents in relation to the
immigration services provided to you in applying for this visa.
You should be aware that KPMG’s registered migration agents may also be concurrently acting as migration agents for your
sponsor if required as part of the application, such as a sponsoring employer, partner or relative.
Although the interests of you and your sponsor are likely to be aligned throughout the course of the engagement, it is possible that
those interests may conflict. KPMG and its registered migration agents are not aware of any present conflicts that would preclude

us from acting for you. If a conflict of interest, or potential conflict of interest arises during the engagement, you will be notified
immediately and KPMG and its registered migration agents will take steps to resolve or manage the conflict as appropriate. In the
event that this occurs, the specific steps to manage the conflict will be dependent on the type and nature of the conflict.

SECTION 3: OTHER IMPORTANT INFORMATION


3.1 LIFE IN AUSTRALIA – AUSTRALIAN VALUES

The Australian government encourages people to gain an understanding of Australia, its people and their way of life, before
applying for a visa to live in Australia. As part of this application every person aged 18 years or over must declare that they will
respect Australian values and obey the laws of Australia. You will be asked to confirm that any person aged 18 years or over, who
is included in this application, has advised you that they agree with this requirement.
The Australian values can be found using the following link.
Further information is contained in the Life in Australia booklet. The booklet is available in a wide range of languages. If you would
like a copy of the booklet it can be obtained from the Department’s web site.

SECTION 4: CLIENT/APPLICANT DECLARATIONS


4.1 INFORMATION ABOUT YOUR DECLARATIONS

• The declarations set out below are required by KPMG, including to meet KPMG’s obligations under the Privacy Act 1988.
• Please read the declarations carefully and confirm your agreement by selecting ‘yes’ (if applicable) to each of the declarations
and by including your signature and the date in Section 4.3 (Signature) in the space provided.

4.2 DECLARATIONS

I certify (on behalf of myself and all persons to be included in the visa application) that:
• I have read and understood the Privacy Statement in Section 1 of this Form and the KPMG Privacy Policy referred to in the
statement. I consent to the collection, use, and disclosure of personal information, including sensitive information, by KPMG.
All other persons included in the CIQ who are aged 18 years or over, have been provided with the Privacy Statement and
have advised me that they also consent to the collection, use and disclosure of their personal information, including their
sensitive information.
