TESTING PERSONNEL

⮚ BEA Representative – He/She shall monitor and supervise the test


administration in the assigned division.
⮚ Regional Testing Coordinator (RTC) – He/She shall oversee the test
administration in the region. He/She shall provide Schools Division
Offices (SDOs) with the list of private schools operating with permit that
are to be included in the test administration. He/She shall prepare a test
monitoring team in the whole region.
⮚ Division Testing Coordinator (DTC) – He/She shall supervise the test
administration in public schools.
⮚ Private Schools Supervisor (PSS) or Division Testing Coordinator
Assistant (DTCA) – He/She shall supervise the test administration in
private schools.
⮚ Chief Examiner (CE) – He/She shall oversee the conduct of the test in
the school. The School Head (SH) shall serve as the CE.
⮚ School Testing Coordinator (STC) – He/She shall assist the CE in the
overall test administration.
⮚ Supervising Examiner (SE) – He/She shall monitor and supervise the
assigned testing rooms. He/She shall conduct ocular inspection the day
before the test administration to be familiarized with the school layout.
⮚ Room Examiner (RE) – He/She shall administer the test in accordance
with the standardized test administration guidelines in the Examiner’s
Handbook (EH).
⮚ Support Staff – He/She shall provide any test-related assistance to the
SH, such as preparation of the testing rooms.

INTRODUCTION

This Examiner’s Handbook is intended for those involved in the


administration of the National Achievement Test for Grade 6 (NATG6). It outlines
the activities to be undertaken at various stages of the test administration. To
ensure a standardized test administration, this Handbook must be strictly followed.

TEST ADMINISTRATION PROPER

The RE shall accomplish the following activities pertaining to this test


namely: Pre-Test, Test Proper, and Post-Test. He/She shall read aloud to the
examinees the instructions in the boxed portion, but read silently the words inside
the parentheses. He/She shall not deviate from this Handbook.

He/She shall check the boxes provided before the boxed instructions after
each activity/instruction has been done/given.

TEST MATERIALS (TMs)

The RE should receive the Test Booklets (TBs) and Answer Sheets (ASs) from
the CE. He/She shall count the TMs received before going to the testing room.

TEST BOOKLETS (TBs). Each examinee will use one (1) TB. It contains the
Examinee’s Descriptive Questionnaire (EDQ) which consists of items on
information about the examinee and the test items in five (5) learning areas:
Filipino, Mathematics, English, Science, and Araling Panlipunan.

ANSWER SHEETS (ASs). Each examinee will use an AS composed of two (2)
pages, with the same serial number printed back to back. The upper portion of the
front page of the first sheet contains the examinee stub and the basic information
about the examinee such as Name, Learner Reference Number (LRN), Birth Date,
School ID, Region and Division Code, School ID, Member of Indigenous People
Group, Sex, Type of School, GASTPE Grantee, Kind of Grant, Special Classes, Third
Quarter Grade, Class Size, Age, Recipient of 4Ps, Name and Address of School.

The ASs are numbered consecutively. There are 27 test items per learning
area. Circles with letters are provided for the answers to each part of the test.

WRITING MATERIALS. Each examinee will use soft lead pencil and sheets
of blank paper for computation purposes.

I. PRE-TEST

1. DELIVERY OF TEST MATERIALS (TMs)


The RE shall receive the following TMs from the CE:

1) Sealed plastic bag containing 30 TBs


2) Sealed plastic bag containing 30 ASs
3) Examiner’s Handbook
4) Examiner’s Transmittal Report Envelope (ETRE) containing the
following BEA forms:
 Forms 1 and 2: List of Examinees and Seat Plan, printed back to
back;
 Form 7: Room Examiner’s Report Form

The RE shall count the TBs in sealed plastic bags in the distribution room.
He/She shall sign the BEA Form 3 (TMs Accounting Form) after accounting
and checking the TMs.

2. ENTRANCE AND SEATING ARRANGEMENT

1. The RE shall be at the assigned testing room not later than 7:00 a.m.
2. The RE shall inspect the seating arrangement before instructing the
examinees to enter the testing room. There shall be six rows by five
columns of armchairs; the RE shall check if the first and last rows are
close to the walls.
3. The RE shall instruct the examinees to sit according to the seat plan.
4. The RE shall instruct the examinees to leave vacant the seat assigned to
an absentee.
5. The RE shall ensure that there shall not be more than 30 examinees in
the testing room.
6. The RE shall admit the examinees following the alphabetical order in the
List of Examinees posted outside the testing room.
7. The RE shall check the identity of the examinees inside the testing room
using the following procedure:
a. Countercheck the identity through the School Identification Card
(ID);
b. If School ID is not available, ask the examinee to present a
notebook or any belonging bearing his/her name;
c. If a & b are not applicable, ask the examinee’s adviser/present
subject teacher to attest the identity.
8. The RE shall instruct the examinees to turn off their digital devices, such
as smart phones and smart watches, and put them in their bags. Their
bags and other personal belongings shall be placed in front, under the
board upon entrance. Only pencils, sharpener, and a clean sheet of
paper are allowed during the test.

(Latecomers should still be admitted to the rooms assigned to them


and they should finish the test at the same time as scheduled for
each learning area.)

3. BOARDWORK
The RE shall accomplish the boardwork as follows. He/She shall note the time
started and time finished for each accomplished activity.

4. ORIENTATION AND GENERAL DIRECTION

Say:

Good morning everybody. I am (State your name). These are some points
you should remember to follow while taking the test. I shall read each one .

Read the following slowly and clearly:

1. You will answer all five learning areas.


2. If possible, you are not allowed to leave the room once the test
has started.
3. Do not open your Test Booklet until you are told to do so.
4. If you have any problem such as missing pages or words that
are not printed clearly, raise your hand so I can help you.

5. Question on the directions or on any test item will not be
entertained after the test has started.
6. Do not write anything on the Test Booklet. A separate Answer
Sheet is provided for your answers. For those answering
Mathematics, you may use a clean sheet of paper for your
computation but submit this to me after the test.
7. Use lead pencil No. 2 in answering the test. Mechanical pencils
are not allowed.
8. Remember to keep your Answer Sheet clean and free from
unnecessary marks. DO NOT fold, crumple, or crease any
portion of it, otherwise it may be rejected by the scanning
machine.
9. Select your answer from the given choices, and blacken the circle
that corresponds to your answer in the appropriate item number
on your Answer Sheet.
10. You may change your answer by erasing it neatly.
11. Work quietly and mind only your own work. Do not use books,
dictionaries, rulers, calculators, smart phones, tablet computers,
smart watches, etc. inside the testing room.
12. Refrain from cheating. If you are caught cheating in any form
such as talking to co-examinees, and have been warned twice,
but persist on cheating, you will not be allowed to take the test
any further.

After you have read the guidelines, allow examinees to go out, if necessary,
before distributing the TMs.

5. DISTRIBUTION OF TMs

Cut with a pair of scissors or a blade one end of the plastic bags (one
containing the TBs and the other the AS). Check their quantities and see to
it that these are accurate and not tampered. In a pack, there are thirty (30)
TBs. Each examinee will have one TB and one AS.

In the distribution of the TBs and AS, follow the numbering of


examinees shown on Page 5. Start with the examinee in Number 1, by
giving him/her the TB/AS with the lowest serial number and end with
examinee in Number 30, giving him / her the highest serial number.

Write on the boardwork under “Orientation and General Direction,


Distribution of Test Materials”, the exact time these activities started and
finished.

Seats of latecomers/absentees should be left vacant. TBs and ASs


allocated to the said examinees should be placed inside the original
plastic bag by the RE pending their arrival. However, the TMs should
not be distributed yet if there are only less than 10 examinees in the
room.

6. CHECKING OF TMs

After each Examinee has received the TB and AS, say:

Everybody, put the Answer Sheet on your chair. (Wait until everybody
has done this.) Look at your Test Booklet. (Pause.) Check the pages one by
one. If you notice any misprint or a missing page, raise your hand so that I
will change it.

Pause. Wait until everybody has done this. Then say:

Everybody, look at your Answer Sheet. Find out if there are defects. In
case there are, raise your hand so that I will change them.

In case a TB or an AS is defective, this should be noted in BEA Form 7


(Room Examiner’s Report Form). When all the TBs and ASs have been
checked, all unused TBs with their corresponding ASs should be placed in
the original plastic bags before the test proper starts. Keep these in your
custody while the test is in progress. Refrain from reading the test
questions, otherwise you will be liable for administrative sanction under
Section 13 of DepEd Order (DO) 55, s. 2016.

7. ACCOMPLISHING OF THE SEAT PLAN

Say:

On the Seat Plan that I will pass, write your name, and the
corresponding Examinee Number and Test Booklet Number.

Pass the Seat Plan as soon as the examinees begin filling in the information
about themselves, not when the test proper is being done. Write on the boardwork
under “Entrance of Examinees, Seating Arrangement”, the exact time the entrance
of examinees and accomplishing the seat plan has started and finished.

8. FILLING IN OF INFORMATION IN THE ANSWER SHEET

(NOTE: FOR ALL INFORMATION TO BE FILLED IN THE AS, PLEASE


REFER TO THE NAME GRID REPLICA)

The Answer Sheet is composed of one (1) sheet printed back-to-


back broken down as follows: The front page contains the circles for the
basic information about yourself. The back page contains the Examinee’s
Descriptive Questionnaire (EDQ) circles numbered 1 to 30. It also
contains circles numbered 1 to 27 for Filipino, 1 to 27 for
Mathematics, 1 to 27 for English, 1 to 27 for Science, and 1 to 27
for Araling Panlipunan. In marking the circle, see to it that the whole
space within the circle is fully shaded.

8.1. NAME
Say:
You shall now fill out your name. Print your name inside the box
provided for on the upper portion of the Answer Sheet. Then print your
name inside each box provided for in the name grid.

Pause. Give the Examinees enough time to write their names. Then
say:

Let us accomplish the name grid found on the upper portion of the
Answer Sheet. The basic rules in filling up the name grid are as
follows:
1. Write only one letter in each box starting with the first box at
the left. Below letter Z are Ñ and dash (-).
2. Enter your last name first, then your first name(s) in the
allotted box and finally your middle initial in the last box. If
you are a “JR", "III", "IV", etc., write it immediately after your
last name but leave one blank box in between.
3. If your last name consists of two or more words such as dela
Cruz, delos Santos, San Ignacio, etc., write your last names
leaving a blank box between. Likewise, if you have two first
names write them leaving a blank box in between.
4. If you have long last names and/or first names, fill your names
up to the last box allotted for your last names and first names.
There are two columns for the middle initial. If you have two or
more middle names write only the first letter of the first and second
middle names (e.g. SJ for San Juan). If you have one middle name,
write the first letter in the first box at the left. Leave it blank for those
without middle initial.

Pause. Let the Examinees follow the instruction, then, say:

If you have filled in your last name, first name, and middle
initial, blacken the circle below each box which corresponds to the
letters written in the boxes.

Go around to see that this is done by the Examinees correctly.

8.2. LEARNER REFERENCE NUMBER (LRN)


Say:

Blacken the circle corresponding to your 12-digit LRN. It consists


of twelve numbers. Check your LRN given by your class adviser.

Go around to check that numbers for the LRN are written correctly in
the corresponding boxes.

8.3. EXAMINEE NUMBER
Say:
The Examinee Number is a bar code found at the upper portion of the
Answer Sheet. (Point to the Bar Code)

8.4. EXAMINEE NO.


Say:

The Examinee No. is a unique six-digit code found at the upper left
portion of the Answer Sheet, below the Bar Code. (Point to the Examinee
No.)

8.5. MARKING INSTRUCTIONS


Say:

● Use No. 1 or 2 pencil only.


● Do not use ink, ball point, or felt tip pens.
● Make solid marks that fill the responses completely.
● Erase clearly any marks you wish to change.
● Make no stray marks on this form.

8.6. BIRTH DATE


Say:

The Birth Date is given in the form of month-day-year,


specifically:
Example: March 6, 2012

Month: Blacken the circle corresponding to the month of birth which is


March.

Day: Two columns are allotted for the day although one circle in each
column is blackened. The range is from 01 to 31. Days ranging from 1
to 9 are entered as 01 to 09, so blacken 0 in the first column and 6 in
the second column.

Year: Only the last two digits of the year of birth are entered. Blacken
1 in the first column and 2 in the second column.

Fill out your birth date in your Answer Sheet following my instructions.

8.7. REGION, DIVISION, AND SCHOOL ID
Say:

Shade the circles corresponding to your Region, Division, and


School ID.

Note: The Region, Division, and School ID must be written on the


board.

8.8. ARE YOU A MEMBER OF INDIGENOUS PEOPLE GROUP?


Say:

Shade the circle corresponding to your response if you are a


member of an Indigenous People Group (e.g. Ibanag, Tagbanwa,
T’boli, etc.).
8.9. SEX
Say:

Shade the circle corresponding to your sex.

8.10. TYPE OF SCHOOL


Say:

Shade the circle corresponding to your type of school.

8.11. ARE YOU A GASTPE GRANTEE?


Say:
Shade the circle corresponding to your response if you are a
GASTPE Grantee.

8.12. IF YES, WHAT KIND OF GRANT?


Say:
Shade the circle corresponding to the Kind of Grant that you
received as a GASTPE Grantee.

8.13. THIRD QUARTER GRADES


Say:

Shade the circles corresponding to your Third Quarter grades in


the five learning areas: Filipino, Mathematics, English, Science, and
Araling Panlipunan.

Note: Each Examinees must have a copy of their Third Quarter


Grades.

8.14. CLASS SIZE
Say:

Shade the circles corresponding to the number of learners in


your class.

Note: Each Examinees must have a copy of their class size.

8.15. AGE
Say:

Two columns are allotted for the age. If you were born on March
6, 2012, you are now 11 years of age. Shade 1 in the first column and
1 in the second column.

Fill out your age in your Answer Sheet following my instructions.

8.16. RECIPIENT OF PANTAWID PAMILYANG PILIPINO PROGRAM (4Ps)


Say:
Shade the circle corresponding to your response if your family
if a recipient of Pantawid Pamilyang Pilipino Program (4Ps).

8.17. NAME AND ADDRESS OF SCHOOL


Say:
Write the name and address of your school on the space
provided.
Note: Each Examinees must have a copy of the name and address of the
school.

8.18. NAME AND SIGNATURE


Say:
At the back, write your name and signature on the space
provided.

Give the examinees enough time to do this. Check that each examinee
does this correctly. Write on the boardwork under “Filling in of information in the
AS”, the exact time the filling in of information in the AS has started and the time it
has finished. Go around to check if the examinees are shading their answers
properly on the ASs.

It is imperative that you check and double check that all examinees have
shaded the correct circles pertaining to the NAME GRID and all the necessary
information about the Examinee. To do this, go around and inspect each
Examinee’s Answer Sheet.

9. ANSWERING OF EXAMINEES’ DESCRIPTIVE QUESTIONNAIRE (EDQ)
Say:

Everybody, turn to Page 1 of your Test Booklet. I will read the General
Directions aloud while you read it silently. (Read the General Directions)

Give the examinees time to read silently with you the General Directions.
Afterwards, say:

Part I is the Examinee’s Descriptive Questionnaire (EDQ) which


asks certain pertinent information about you. It is numbered 1 to 30 and
should be answered in the space provided for.
Part I will last for 25 minutes. Everybody, open your Test Booklets
to the EDQ.
(Show them the EDQ portion), Ready… Begin!

Write on the boardwork under “Answering of Examinees’ Descriptive


Questionnaire”, the exact time the EDQ has started and finished. Go around to
check if the examinees are shading their answers properly on the ASs.

After answering the EDQ, say:

STOP! Let us go to the test proper.

II. TEST PROPER

1. ADMINISTERING THE TEST IN FILIPINO


Say:

Everybody, get your Test Booklet. Turn to the page where Filipino
starts. You will answer this test within 30 minutes. You may now begin
answering the test.

Write on the boardwork under “Part II. Test Proper, Filipino”, the exact time
the test proper for Filipino has started and finished.

2. ADMINISTERING THE TEST IN MATHEMATICS


Say:

After answering Filipino, turn to the page where Mathematics starts.


You may use a clean sheet of paper for your computation but submit this to
me after the test. You will answer this test within 40 minutes. You may now
begin answering the test.

Write on the boardwork under “Part II. Test Proper, Mathematics”, the exact
time the test proper for Mathematics has started and finished.

3. ADMINISTERING THE TEST IN ENGLISH
Say:

After answering Mathematics, turn to the page where English starts.


You will answer this test within 30 minutes. You may now begin answering
the test.

Write on the boardwork under “Part II. Test Proper, English”, the exact time
the test proper for English has started and finished.

4. ANNOUNCING THE SNACK BREAK


Say:

You are given fifteen-minute (15) break for your snack. Insert the
Answer Sheet in your Test Booklet and secure them properly. For those who
brought snacks with them, you may eat in your respective seat, but be
careful not to smudge your Test Booklet and Answer Sheet.

Write on the boardwork under “Snack Break” the exact time started and time
finished.

5. ADMINISTERING THE TEST IN SCIENCE


Say:

Everybody, get your Test Booklet. Turn to the page where Science
starts. You will answer this test within 30 minutes. You may now begin
answering the test.

Write on the boardwork under “Part II. Test Proper, Science”, the exact time
the test proper for Science has started and finished.

6. ADMINISTERING THE TEST IN ARALING PANLIPUNAN


Say:

After answering Science, turn to the page where Araling Panlipunan


starts. You will answer this test within 30 minutes. You may now begin
answering the test.

Write on the boardwork under “Part II. Test Proper, Araling Panlipunan”, the
exact time the test proper for Araling Panlipunan has started and finished.

Note: The unused Scannable Answer Sheets shall be collected by the


SE/CE at the time when the Examinees are answering the last learning area.

III. POST-TEST

1. RETRIEVING OF TMs AND DISMISSING THE EXAMINEE/S

Say:

Insert the Answer Sheet in the Test Booklet and submit them to me as I go
around to collect the test materials.

Collect and count all used and unused TBs and ASs. Used Scannable ASs
are arranged consecutively by Examinee Number and place it in the original plastic
bag. Unused ASs shall be turned over to the CE. Examinees may leave the room
only after accounting of TBs.

After all the TMs are accounted, say:

We are now done with the test. Thank you very much. You may now leave
the room.

Write on the boardwork under “Retrieval of TMs and Dismissal”, the exact
time the retrieval of TMs and dismissal of the examinees has started and the time it
has finished.

2. PREPARING THE REPORTS AND SEALING THE ETRE

The RE shall prepare the following contents of the Examiner’s Transmittal


Report Envelope (ETRE).

ETRE
1. Used Scannable Answer Sheets (arranged consecutively by Examinee
Number; placed in the original plastic bag)
2. Form A: List of Examinees (posted on the door)
3. BEA Form 1 & 2: List of Actual Examinees & Seat Plan, printed back
to back
4. BEA Form 7: Room Examiner’s Report Form
5. Boardwork with actual time record (posted on the board)

The RE shall seal the ETRE while still inside the testing room and shall affix
his/her signatures thrice (3x) across on the flap of the ETRE upon sealing it.

3. TURNING OVER OF ETRE TO THE CE


The RE shall turn over the ETRE to the CE. The Chief Examiner’s
Transmittal Report Envelope (CETRE) shall contain the following documents:

2. BEA Form 3: Test Materials Accounting Form
3. BEA Form 4: Chief Examiner’s Report Form
4. School Header

4. ACCOUNTING OF TMs BY THE CE
The CE shall account the TMs in accordance with the following:
a. Collect and account all ETREs assisted by the SE (if any).
b. Arrange and bundle the TMs accordingly in terms of the following:
 The ETREs should tally with the number of testing rooms. A report
on the number of ETREs should accompany the bundle.
c. Return to their original plastic bags all used and unused TBs with serial
numbers arranged consecutively. These should be resealed by stapling
the open end of the plastic bag.
d. Place inside the CETRE the unused Answer Sheets, BEA Form
3, BEA Form 4 and School Header.
e. Place BEA Form 5 (Test Booklet Quantity and Completeness Verification
Sheet) in Box 1 of the Test Booklets.
f. Place BEA Form 6 (Answer Sheet Quantity and Completeness
Verification Sheet) in Box 1 of the Answer Sheets.

TO THE DIVISION TESTING COMMITTEE:


This Examiner’s Handbook MUST BE STORED IN THE TESTING CENTER
after the test together with the Name Grid Replica for future use.

SECTION 13: Breach of Security in National Examinations


and Corresponding Sanctions
The security and integrity of test materials shall be upheld in the whole process of
test administration. This Order rescinds DECS Order No. 85, s. 1999, “Breach of
Security in National Examinations and Corresponding Sanctions” and DepEd Order
No. 47, s. 2015, “Inclusion of Acts as Violations to National Examinations.” The
following acts are deemed as violation/infractions of security pertaining to national
examinations in addition to:
1. Reading of test booklets other than by the examinee
2. Supplying answers to examinees
3. Cheating
4. Plagiarizing
5. Loss of test booklets
6. Photocopying of test booklets
7. Capturing test items through electronic gadgets
8. Distribution and posting of photos of the test booklet in any form of media
9. Opening the test materials prior to testing schedule
10. Late submission of test booklets and/or answer sheets
11. Tampering of examinees’ answer sheets
12. Infringement of copyright
13. Allowing impostors and substitute examinees
14. Misrepresentation with regard to age, residence, and status
15. Failure to closely monitor test procedures resulting in test irregularities

Any or all of the following sanctions will be imposed on any or all of the
aforementioned violations committed by those involved:

1. Examinees
The degree of violation and corresponding sanction shall be determined by the
Chief Examiner.

a. First and second offenses: reprimand


b. Third offense: Cancellation of test papers / invalidation of test results

2. Testing Personnel
The degree of violation and corresponding sanction shall be determined by the
Schools Division Superintendent.

4. Suspension of salaries and incentives/benefits of those concerned for one


to three months
5. Work suspension
6. In the case of infringement of copyright, legal remedies will be considered.
