last September 8, 2023.

2. Conducted School Mathematics Club election last September 12, 2023.

3. Attended the Division Conference of SHS Assistant Principals and Focal


Persons on September 19, 2023 at San Juan National High School, San Manuel,
Pangasinan.

4. Attended the Division Training-Workshop on Revisiting the Learning Action


Cell (LAC) as a Continuing Professional Development Strategy for Master
Teacher and Key Teachers on September 25-27, 2023 at CB Mall Event Center,
Urdaneta City.

5. Conducted the School-based National Statistics Month (NSM) celebration


last September 22, 2023.

6. Administered the Diagnostic Test last September 27 and 28, 2023 and
submitted the required reports (Mean, MPS, SD) on time.

7. Participated in the Division National Statistics Month (NSM) celebration last


September 29, 2023 at San Quintin National High School, San Quintin,
Pangasinan.

8. Administered the 1st Quarter Exam last October 26 and 27, 2023 and
submitted the required reports (Mean, MPS, SD) on time.

9. Conducted home visitation to students encountered more absences and failed


to submit their activity sheet on time.

1. Conducted PTA Meeting cum 1st Quarter Card Day on November 13, 2023.
2. Attended the Division Conference of SHS Assistant Principals and Focal Persons featuring
Division Launching of Project T.I.B.A.Y (Target Independent Readers with resilient well-
Being And Youth Models) on November 14, 2023 at Alcala Training Center, Alcala,
Pangasinan.
3. Attended the Division Capacity Building of Grade 9 & 10 English, Science, and
Mathematics Teachers on the Implementation of the National Learning Camp for the School
Year 2023-2024 on November 15-17, 2023 at CB Mall Sports House, Urdaneta City.
4. Served as Chairman on Program Invitation during 2023 Municipal Meet held on November
29, 2023 at Panganiban NHS, Tayug, Pangasinan.
5. Served as Chairman of Technical Committee and Resource Speaker during School-based
GAD Training Focused on Empowering Family: Approach to Responsible Parenthood held
on November 25, December 2 and 8, 2023.
6. Participated in Cluster V Recreational Mathematics Games Festival at Umingan NHS,
Umingan, Pangasinan on December 7, 2023 and won 2 nd Place in Tower of Hanoi (Teacher’s
Category).
7. Served as Coach of the Winners during the Cluster V Recreational Mathematics Games
Festival at Umingan NHS, Umingan, Pangasinan on December 7, 2023.

10.Served as Chairman on Program Invitation and Certificates during Fellowship Day 2023
and Induction Program held on December 13-14, 2023.
11. Served as Vice-Chair of Planning Committee and one of the resource speakers during
INSET on Multimedia Approach: A Teacher’s Effective Way to Foster Learning last January
24-26, 2024.
12. Assisted the School Head in crafting the SHS Class Program 2nd Semester SY 2023-2024.
