Purposes:
1. To establish a baseline data for subsequent evaluation.
2. To identify whether the pulse rate is within normal range.
3. To determine whether the pulse rhythm is regular and the pulse volume is appropriate.
4. To determine the equality of corresponding peripheral pulses on each side of the body.
5. To monitor changes in the clients health status
6. To monitor clients at risk for pulse alterations (e.g. those with a history of heart disease or
experiencing cardiac arrhythmias, hemorrhage, acute pain, infusion of large volumes of
fluids, fever, etc.)
7. To evaluate blood perfusion to the extremities.
Assess:
1. Clinical signs of cardiovascular alterations such as dyspnea (difficulty of breathing), fatigue
(exhaustion), pallor (paleness), cyanosis (bluish discoloration of the skin and mucus
membranes), palpitations (tremors or vibrations), syncope (fainting), or impaired peripheral
tissue perfusion (as evidenced by skin discoloration and cool temperature.)
2. Factors that might alter pulse rate. (e.g. emotional status and activity level)
3. Site most appropriate for assessment.
