Step-by-Step Teacher Instructions

REQUIREMENTS:
 Classroom Internet Access
 1:1 Electronic Devices (computers, laptops, cell phones, iPads/tablets etc.)
 BOTH Teacher and Students must have a Google Account.
(Check that students know their Username and Password to access their Google
Account at least 1 day prior to using this resource.)

STEP 1: Access your Google Drive resource by double-clicking on the image below.

STEP 2: Click on “Make a copy” to save a copy of the document to your Google
Drive.

STEP 3: Modify the settings to meet your assessment needs.

The default settings for this Form are shown below.


STEP 4: Modify the ANSWER KEY settings to meet your assessment needs.

Suggestions:
Add other answer choice options.

Toggle “Required” ON/OFF

Change or omit point values for each question.

Add Answer Feedback to student responses.


STEP 5: Send the Google Form to your students by clicking “SEND.”

STEP 6: Select the link icon and check Shorten URL to obtain a user-friendly link.

STEP 7: Click on “RESPONSES” to view the Class Summary as well as Individual


Student responses.
