TRAFFIC INVESTIGATION REPORT

NATURE OF THE CASE : RECKLESS IMPRUDENCE RESULTING IN DAMAGED


TO PROPERTIES.

DATE/TIME/PLACE : On April 27, 2015 at about 7:40 o’clock in the morning,


along OG Road, at the vicinity of Barangay Mapalad, Arayat,
Pampanga.

VEHICLES/DRIVERS INVOLVED

: V1-FUSO CANTER DROPSIDE, with plate number TTV-


131, being driven by Dionisio Elias Ador y Baja, 76 years old,
married and a resident of Malapit San Isidro, Nueva Ecija,
holder of PDL with number C05-78-077288, valid up to 2015-
11-05.

: V2- Daewoo Bus (Arayat Express), with plate number


CXV-970, driven by Fernando Vinuya y Mallari, 50 years old,
married and a resident of Barangay San Nicolas, Arayat,
Pampanga, holder of PDL with number C10-84-088564 valid
up to 2016-09-05.

FACTS OF THE CASE:

This particular case was reported to this office by personnel of PAAC at about
8:00 o’clock in the morning of April 27, 2015, upon receipt of the report elements this
office together with the undersigned investigator immediately proceeded at the place of
incident to verify the veracity of the report and conducted an investigation, upon arrival
at the place of incident noted that the vehicles involved were on their original position,
photographs were taken and rough sketch was also prepared by the investigator duly
signed by both drivers, likewise, the involved drivers and other witnesses were
interviewed relevant thereof and based on their oral statements the following facts were
established to wit;

Investigation conducted disclosed that prior to the incident both vehicles involved
(V1 and V2) were cruising on the same direction coming from Nueva Ecija heading
towards the direction of Arayat town proper wherein V2 was then moving ahead of V1,
upon reaching at a certain point, V1 failed to maintain proper distance and
miscalculated, at that instance, the rear back portion of V2 was accidentally hit and
bumped by the frontal portion of V1 which was tailing from behind, as a result both
vehicles involved incurred still undetermined amount of damages. No reported injuries
on the drivers and their respective occupants.

ACTION TAKEN: Case was investigated


STATUS OF THE CASE: Under Investigation, both parties are on the process of negotiation
for the possible settlement of the said incident.

TRAFFIC ACCIDENT REPORT

NATURE OF CASE: RECKLESS IMPRUDENCE RESULTING IN HOMICIDE AND


DAMAGE TO PROPERTIES.

DATE/TIME/PLACE : December 12, 2015, at about 6:20 PM, along OG Road, at the
vicinity of Barangay Plazang Luma, Arayat, Pampanga

VEHICLES/DRIVERS INVOLVED:

V1- Isuzu Truck, with plate no. RDL 552, driven by Danny
Dela Cruz y Balae, 44 years old, married and a resident of Dampulan Jaen Nueva
Ecija, holder of Temporary License-PDL with no. CO5-03-030986 valid until 06-02-2018

V2- Kawasaki Fury, red and gray in color with plate number
1970 TF, driven by Jose Paolo Lopez y Bondoc, 27 years old, single, and a resident
of Purok 6 Brgy San Nicolas, Arayat, Pampanga, Holder of Non-PDL with nr.
N2507003361 valid until 3-12-18.

WITNESS/ESS: To be presented later

FACTS OF THE CASE:

This Particular case was reported to this office by a concerned citizen at


about 6:30 pm of same date, upon receipt of the report, elements of this office together
with the undersigned investigator immediately proceeded at the said place of
occurrence to verify the veracity of the report and to conduct an investigation, reached
thereat, involved vehicles were already removed and no longer at their original position,
likewise, photographs were taken, driver involved and other witnesses were interviewed
relevant to the said accident and thru their oral statement, following facts were gathered
to wit;

Initial investigation conducted disclosed that on the given time, date and
place of occurrence, vehicles involved were then cruising on the same direction heading
towards the direction of Cabiao, Nueva Ecija. V2 moving ahead while V1 coming from
behind, upon reaching at a certain point V1 overtook V2 and while on the process of
overtaking and seeing another vehicle from opposite direction going to Sta Ana,
Pampanga, the driver of V1 decided to swerved his driven vehicle to the right but
accidentally sideswipe V2, as a result, driver of V2 fell down on the cemented road and
sustained fatal head injury and immediately brought to Arayat District Hospital, Arayat
Pampanga for medical treatment and later transferred to JBL Hospital, City of San
Fernando Pampanga for possible operation and confinement. Both vehicles involved
incurred undetermined amount of damages. The driver’s license of both drivers were
temporary confiscated as well as the OR/CR of the vehicles involved. The driver of V1
was arrested and brought Arayat Municipal Police Station for proper disposition, while
both vehicles involved were impounded.

At about 2:30 AM of December 13, 2015 driver of V2 was expired at the said
hospital while undergoing medical treatment. A criminal complaint of Reckless
Imprudence Resulting in Homicide and Damage to Properties is now being prepared by
the undersigned investigator for filing at the Provincial Prosecutor Office, City of San
Fernando, Pampanga thru inquest proceedings.
