2. NUTRITIONAL-METABOLIC PATTERN
Patient’s daily food intake usually contains rice, vegetables, fried/dried fish, and
conservatives, such as noodles, churiso, etc. He also, takes his medication. He drinks
at least 8 glasses of water in a day and a glass of milk every morning. He verbalized
that he doesn’t have any swallowing problems. He gained weight after losing weight
when he was admitted, because of stroke last 2018. Patient’s skin appears to be dry.
3. ELIMINATION PATTERN
Patient stated that he voids at least 4 times in a day. Urine is yellow and not cloudy.
Patient defecates at least once every morning. Stool is brown and soft. He also,
experience no difficulty or discomfort when defecating and voiding.
4. ACTIVITY-EXERCISE PATTERN
Patient does simple exercises at home like, walking, stretching and is able to do self-
care independently, like grooming, bathing, toileting, feeding and dressing. Patient do
watch television or walk around the house during spare time. Functional level code:
Level 0: Full self-care
5. SLEEP-REST PATTERN
Patient states that he doesn’t experience difficulty in sleeping. He usually sleeps at 10
PM and wakes up at 5 to 6 AM. He also takes afternoon naps at least once.
6. COGNITIVE-PERCEPTUAL PATTERN
Patient clearly hears. For vision, patient still see faces and surroundings, but will need
to wear eyeglasses when reading. Patient exhibits signs of memory loss. He
confusedly answered some of the questions asked and responds late at some times.
7. SELF-PERCEPTION-SELF-CONCEPT PATTERN
Patient stated that before, he was still able to do a lot of things compare to when he
was diagnosed with stroke, his activities were limited. He said that, even when he had
stroke, he didn’t lose hope and still believes that he will get better soon.
8. ROLE-RELATIONSHIP PATTERN
Patient lives with his extended family. Accordingly, they handle family problems by
talking with each other. The patient also, stated that he sometimes cannot control the
things he says when he’s angry during family discussions. His family, especially his
wife and son are hands-on and are the ones taking care of him, helping him take his
vital signs and medication every day.
9. SEXUALITY-REPRODUCTIVE PATTERN
Patient is married and living with his wife. They have 4 children in total.
