GENERAL DIRECTION: The interview questions listed herein aim to substantiate the plans, systems, structures, and facilities
contained/mentioned in the LGU official documents submitted during the earlier stages of the SGLG assessment. Respondents
must also be able to explain how actual processes take place sufficiently. Note that these are the minimum questions, and the
Validator is highly encouraged to pose subsequent/follow-up queries that could further demonstrate the LGU’s current condition.
For barangay-level verifications, the interviews may feature only one barangay, or the city/municipality may highlight multiple
barangays. This form is for SGLG Validator’s use only and should not be distributed to LGUs. The Validator may also translate the
questions to the local language whenever necessary. This form is to be submitted along with Form 6.
Part A. Evacuation Management Systems and Resources
This guides the Validator for the interview with a Punong Barangay on evacuation management. The
interview may be conducted during the on-site inspection of the evacuation center.

ENRO). The officer may also be assisted by other officers who can discuss their EM initiatives. If time permits,
the interview may be supplemented with on-site visit to the EM facilities in the LGU.

passed per Regional Assessment.
