LETTER OF CONSENT

To whom it may concern:

I, the parent/guardian of __________________________________, do hereby give my full


consent for his/her to participate in the JOINT BSP-GSP CAMP organized by Tagbilaran City
Science High School Scouts Organization to be held this coming December 9-10, 2022 at the
Tagbilaran City Science High School Campus.

I have considered the benefits that my son/daughter will derive from his/her participation in this
activity with the understanding that the minimum health standards shall be implemented and that
every precaution shall be taken to ensure his/her safety.

I shall not hold the Bohol Girl Scout Council, Bohol Boy Scout Council, Tagbilaran City
Science High School or its representative responsible for any untoward incident that may happen
beyond their control.

This letter of consent is being issued this 7th day of December, 2022 at Tagbilaran City Science
High School.
