I. Instructions: Please rate the strengths and weaknesses of the HOST TRAINING
ESTABLISHMENT (HTE) and your IMMEDIATE JOB SUPERVISOR in terms of meeting
your needs as a practicum student. Use the following scale:

Excellent = 5 Marginal = 2
More than adequate = 4 Poor = 1
