Name of the Project : Tables and Chairs
Date : July 13, 2023
Appropriation : One Hundred Thousand (P 100, 000.00) Pesos.
Rationale :
Provincial Federation of Barangay Health Workers of Surigao del Sur it has 17
municipalities & 2 cities has 3,584 registered and accredited Barangay Health Workers of
Urban and Rural Barangays, that render the vital function in their respective communities by
providing assistance and support to the health workers of the Provincial Health Office and
provides IEC and motivation services for Primary Health Care. They are considered the
guardians of the communities’ health they are serving.
Consequently, in support for the project proposal of BHW’s apart from their rating
participation spirit of voluntarism, there is need to provide them livelihood such as Tables
and Chairs.
Objectives:
1. Provide a sustainable additional table, chairs for livelihood ; extra funds for the
federation of BHW in Surigao del Sur

This proposal, once accepted, will become effective and useful for all BHWs of the
entire Province of Surigao del Sur. Those purchases may be a big help not only for the
officers but also to fellow BHWs.

RESOLUTION No. 001 – 23
A Resolution for P.B.I. Performance based incentive awarded to Provincial Federation
of Barangay Health Workers of Surigao del Sur
Whereas, the money worth P 100,000.00 pesos is budgeted for the Barangay Health
Workers Federation of Surigao del Sur to help the federation to enhance, effective and
recognize BHWs of the province through Project Proposal with the help of P.B.I by
Department of Health-Caraga Region Office;
Whereas, through series of activities, the BHW federation Officers has able to identify their
needs and agree on appropriate solutions;
Whereas, the Province of Surigao del Sur BHW Federation of Barangay Health Workers are
composed of 2 cities & 17 municipalities barangays; 3,584 of barangay health workers.
Whereas, among those solutions they have successfully prepared a project proposal named;
purchase of tables, chairs.
Whereas, this proposed project has a total cost One Hundred Thousand Pesos (Php
100,000.00);
Wherefore, upon mass motion and seconded in mass, BE IT RESOLVED, as it is hereby
resolved by the Barangay Health Workers Federation of Surigao del Sur., in meeting
dully assembled – to approve the proposed project of the BHW Federation.
Resolved Further, to furnish of the Resolution for the Project Proposal Procurement of
purchasing tables, chairs..
