Case studies provided below belong to three Yin diseases according to Shang Han Lun’s 6-
channel diagnosis. State the 6-channel diagnosis, pathogenesis, treatment principle, formula
and ingredients with dosage for one of the case studies provided below. Discuss your answer
with related Shang Han Lun’s clause.
1. A 70-year-old woman had suffered for several years from stomach pain which was
worsened whenever she caught the common cold. The stomach pain came and went
during the day and eased when she went to bed with thick warm blanket. Her appetite
was normal but preferred warm cooked food. She usually had loose stool after
consuming cold and raw food. Her pulse was soggy and moderate. Her tongue is pale
with thin white coating.
Xiang Sha Liu Jun Zi was administered to warm the Spleen and regulate the Qi.
However, the pain was only temporarily alleviated, and the condition relapsed a few
days later.
2. A 48-year-old male with a history of hypertension for over ten years presented with
severe dizziness aggravated by standing up. His face was flushed, and he suffered
from profuse cold sweats, palpitations and vexation. His appetite is less, and he has
loose stool with visible undigested particles. He has cold extremities but aversion to
heat; his pulse was submerged, threadlike and faint. His tongue was pale and swollen.
3. A 20-year-old girl had suffered from frostbite each winter for many years. The
symptoms included coldness, numbness and pain in the hands and feet and itching and
swelling at the site of the chilblains. She also complained that her period volume was
scanty, period cycle was usually delayed and dysmenorrhea during the first and
second day of period which can be relieved by warmth. Her pulse was faint and
threadlike, her tongue was pale with a thin white moist tongue coating.
4. A 51-year-old male had suffered from recurrent diarrhoea for over two years. The
number of episodes ranged from 2 to 6 times a day, each time accompanied by
borborygmus, abdominal pain and there was white mucus and blood in the stool. He
presented with fatigue, poor appetite, cold extremities, pale and swollen tongue with
tooth marks, and a submerged, threadlike and weak pulse. The Western diagnosis was
ulcerative colitis.
5. A male patient had contracted a head cold, for which he was prescribed a cool acrid
formula. His condition did not improve and more than ten days later, he had a severe
headache. His symptoms included aversion to cold in the afternoon, nose blockage
with white discharge, sneezing, a heavy aching sensation in the body and a lack of
thirst. His tongue coat was white and slippery and the pulse was submerged, faint and
forceless.
6. A 21-year-old male contracted the flu two weeks ago. The fever receded after
consuming conventional medicine. However, he did not recover fully and complained
of palpitations, shortness of breath, an oppressed sensation in the chest, fatigue and
excessive sweating. The symptoms were all worse at night. His face was red and he
suffered from feverish sensation, a dry parched throat, vexation and insomnia. The
edge and tip of his tongue were red, the tongue coat was scant, and the pulse was
threadlike and rapid.
