Instructions for Return Demonstration

1. Language to be used: Practice delivering in English or Tagalog since the language will depend on the
level of understanding of the client. If the client is more comfortable talking in Tagalog, the Nurse
may use Tagalog.
Note: Make sure to translate your Gordon's questionnaire in TAGALOG.

2. Purpose of the assessment: Wellness consultation/routine health check-up.


Chief complaint – during the time of interview and the patient is well, the reason for seeking
health care should be documented as "for general physical assessment" or for "consultation"

3. Coverage for the actual Return Demonstration: Health History Taking/Preparing the environment.
Show or perform, if not possible due to inevitable circumstances, at least mention.
a. Initiation: Follow the sequence in the e-tool
b. Body: Draw lots for the 11 functional health patterns will be done and focus on the component
you got. Follow the sequence in the e-tool.
c. Closing: Follow the sequence in the e-tool.
d. Setting of contract - set contract with the patient as to how long you will take care of him/her. If
it is for wellness consultation, you will only take care of her/him for one day. Hence, in the
closing - you may state "Tatawagan ko nalang Po kayo kung may kinakailangan pa po akong
datos".
e. Both verbal and non-verbal communication skills will be observed by the CI, hence no need to
mention.
f. GSISA, Head to neck, Chest to abdomen, Musculoskeletal assessment: Make sure you have
performed each step in the e-tool, sequentially, and if not possible to perform due to
unavoidable circumstances, at least mention the step to your CI.
Note: a. The interview will focus on Gordon's Functional pattern. It will not start with
biographic data.
b. Note taking can be written or recorded via cellphone (ask permission from the client).

4. As to the environment, it should be acted. Example: Show that you draw the curtain to
provide privacy.

5. Coverage for documentation: A separate formatted file will be given to all sections of what you need
to gather. Interpretations and basic analysis will be included in the final documentation to be
submitted 1 week after the actual Return Demonstration of the student.

6. Time allocation: please check the revised study plan for the time allocation for actual Return
Demonstration (4th column). Current revision will be shared by your assigned CI.

7. E-tool: a copy will be provided to the students. Total grades per procedure will be reflected both in
hard copy and canvas.

8. Additional instructions and clarifications will be given for succeeding procedures, if and when there
will be changes due to unavoidable circumstances e.g. strike, sudden announcements etc.
