Association, Inc. (“THRHAI”) depose and say that:
1. I am the contractor/sub-contractor that will undertake the construction/renovation/repair works on Block

2. I have fully informed the homeowner regarding the THRHAI Village Rules and Regulations (“VRR”) and
construction related circulars issued by the THRHAI, and that the latter understood the same,
3. I am obliged to abide by the VRR throughout the performance of the construction project for said
Homeowner at said Block and Lot
4. I am amenable to the imposition of fines, penalties, and/or other charges for any infraction to the VRR
and other construction related circulars issued by the THRHAI,
5. I bind myself to fully inform and appraise the Homeowner of any fine, penalty, and/or other charges on
his/her construction bond or in any way prejudicial to his interest.

construction project at above mentioned Block and Lot. We consent to THRHAI’s possible disclosure with relevant
third parties all personal data only when it is necessary in the performance of our requirement or as may be
required by legal authority or by law. We further consent that THRHAI will retain our personal data for a
reasonable period to deal with any matter that may arise in conjunction with our Clearance To Construct
application in accordance with THRHAI’s Privacy Policy. We understand that THRHAI implements reasonable
security measures to protect and to keep our personal data confidential. We are responsible for ensuring that our
personal data is correct and true. Should we wish to access or to amend our personal data or withdraw our
