In line with the recent incident, blasting with fuel release, of the installed electric transformer located at the
main gate of the school, the provider PANELCO with the secondary recommendation of the Bureau of Fire
& Protection -San Nicolas Pangasinan the electric supply of the whole school was disconnected and will
only be revive once the transformer is replaced.
May we therefore request for the immediate approval to proceed with the rental of the 50KVA transformer to
be supplied by PANELCO III, this is to address the immediate need of the school’s electric supply. As the
current transformer is no longer usable and is already busted.

WHEREAS, on 17th of April, the current transformer installed at the main gate supplying the
electricity of the school were busted and damaged, causing the electric provider PANELCO with
the recommendation of Bureau of Fire & Protection – San Nicolas, Pangasinan to shut down the
whole electric supply of the school;
WHEREAS, on 18th of April, Bids and Awards Committee received a letter of end user requesting
for an immediate approval of the rental of the 50KVA transformer from PANELCO to replace the
busted and damage transformer in order to continue the operations of the school;
WHEREAS, the request for an immediate rental of transformer is necessary to continue the
operations of the school and for the health and safety of school personnels and students particularly
that ventilation with the help of electric fans is needed due to the rise of temperature brought by the
summer season;
WHEREAS, the BAC Section 7.2, Rule II of the 2016 Revised Implementing Rules and
Regulations of Republic Act No. 9184otherwise known as the Government Procurement Reform
Act (GPRA) states that: “No procurement shall be undertaken unless it is in accordance with the
approved APP, including the approved changes thereto. The APP must be consistent with the duly
approved yearly budget of the Procuring Entity and shall bear the approval of the HoPE or second
ranking official designated by the HoPE to act on his behalf”;
WHEREAS, Sections 53 and 53.2, Rule XVI of the GPRA further provides that:

NOW, THEREFORE, We, the Members of the Bids and Awards Committee, hereby RESOLVE
to recommend to the Head of the Procuring Entity the following:
b) to authorize the Supply Officer to proceed with the above-mentioned item, pursuant to
Annex H(IV)(J) of the Implementing Rules and Regulations of RA 91841 and submit to the
HoPE its recommendation for the issuance of the award and notice to proceed

Sir,
We would like to present this Letter of Intent to signify our interest in leasing a 1(one) unit Electric
Transformer, 50KVA as a replacement for the current electric transformer which was damaged last
April 17. The said equipment will be installed at the front gate of the San Nicolas National High
School, N. Patrico St. Poblacion East San Nicolas Pangasinan.
The intent is to lease the said equipment for a period of 6(six) months while we are waiting for the
instruction of division office re- Electrification Program.
We are hoping for your immediate response on this request, as the health and safety of our
Teaching; Non-Teaching Personnels and Students matters most.
