I, EUGENIO LOPEZ, of legal age, Filipino, widowed, resident of Brgy.Talisay, Lipa City,
after having been sworn  in accordance with the law hereby depose and state that:

1. I personally know IMELDA A. MENDOZA, who is my long time neighbor at


Brgy. Talisay, Lipa City, and the fact of her birth is registered at the Local Civil
Registrar of Lipa City;

2. I know for a fact that the parents of IMELDA A. MENDOZA are AURELIO E.
ARELLANO and GABINA A. LOBO as appearing in her Certificate of Live
Birth;

3. I am not in any way related to any of the foregoing persons within the fourth civil
degree either by consanguinity or affinity, nor do I have any pecuniary interest in
this matter whatsoever;

4. I am executing this affidavit to attest to the truthfulness of the foregoing


statements and to establish the names of parents of IMELDA A. MENDOZA and
for all legal intents and purposes it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 5th day of September
2022 at Lipa City, Philippines.
