1. Reference: Blotter Entry No. 2022-12-00119 of page 028 dated 23 December
2022,

2. This pertains to the vehicular traffic incident Reckless Imprudence Resulting in


Serious Physical Injuries and Multiple Damaged to Property that transpired at around 7:50
PM of December 23, 2022 along the provincial road at Barangay San Isidro Sur, Luna,
Apayao involving (V1) a Honda geneo/MC, color blue/white with MV file # 0201-1007935
driven by John Rainier Peru y Tabling, 19 yrs old, student, native of Poblacion, Kabugao,
presently residing at Poblacion Luna, Apayao; (V2) a color white Isuzu crosswind bearing
plate # WOU 278, driven by Prince Hollywood Apostol Al Sakkaf,male, 31 yrs old, married,
businessman, with DL No. P06-22-000117and a resident of Brgy Tumog, Luna Apayao;
(V3) a color black Ford Ranger/pick-up bearing plate # AAJ 8653, driven by Camille Joy
Agcaoili y Caoili, 25 yrs old, single, Pharmacist and a resident of Juction Libertad, Abulug,
Cagayan; and (V4) a NV350 Urvan 2.5, color metallic brown bearing plate # CAK 8947,
driven by Rosvin John Tumbaga y Calizo, 27 yrs old, single, Engineer, with Professional
Drivers License No. P06-14-000047 and a resident of Poblacion, Pudtol, Apayao.

3. Initial Investigation disclosed that V2 was traveling along the said road coming
from Far north Luzon General Hospital and training center, brgy. Quirino, Luna, Apayao
proceeding towards Tumog, Luna, Apayao while V1 came from San Isidro Sur, Luna,
Apayao and suddenly turned left proceeding to San Isidro Norte, thereby the upcoming
V2 collided with V1. As a result of the incident, the driver of V1 was thrown and lying
facing down on top of the windshield of V2 while said motorcycle was thrown at the
opposite lane and its debris were scattered along the said road. V3 that was also traveling
from San Isidro Sur proceeding to Junction, Luna, Abulug, Cagayan tried to evade from
colliding with V1 and its debris that were scattered, however V3 still collided with V1 and
subsequently bumped the concrete bench and Material Recovery Facility (MRF) beside
the said road. Likewise, V4 that was also travelling from Pudtol, Apayao following V3 and
proceeding towards Junction Luna, Abulug, Cagayan also tried to evade from colliding
with V1 and its debris, however V4 still collided with the debris of V1.

4. Driver of V1 sustained multiple fracture on both legs and was brought to Far
North Luzon General Hospital and Training Center by the responding members of BFP
and PDRRMO for medical treatment. Drivers of V2, V3, and V4 were fortunately unhurt.
V1 totally wrecked while V2 incurred damages on its hood, grill and bumper. V3 incurred
damages on its hood, left headlight and front left side while V4 also incurred damages on
