LETTER AGREEMENT

We take pleasure in accepting your offer to provide the GESU EUCARISTICO ACADEMY
on an EMPLOYMENT WITH A FIXED PERIOD as FULL TIME BASIC EDUCATION TEACHER from
Sept 1, 2021 until May 31, 2022 under the following terms and conditions.

1. Your services will be utilized on a FULL TIME EMPLOYMENT BASIS to report for EIGHT (8)
HOURS every day from Monday thru Friday. SIX (6) HOURS of Academic Teaching and TWO (2)
HOURS of documents work.

2. Your details of duty are those provided by the SCHOOL ADMINISTRATION other than those that
may be specifically given to you by the PRINCIPAL from time to time who shall also evaluate your
work performance and attitude every now and then as part of the school’s human resource
development program.

3. You shall abide and adhere strictly to all Rules and Regulations of the school and those that may be
imposed from time to time and any violation thereof shall be a cause for the termination of this
LETTER AGREEMENT after due process.

4. That for and in consideration of your services, you will be paid the amount of eight thousand pesos
only (Php. 10,000) PER MONTH with the following breakdown: basic rate - 8, 500, allowance -1000
and incentives 500 pesos.

5. You shall promptly notify the ADMINISTRATION of any grievance or claim that you may have, and
anything not reported shall be deemed to have been waived and shall expire with the agreement.

6. The attached PERSONAL DATA SHEET shall form part of this LETTER AGREEMENT and any
falsification or omission of any vital information shall be considered violation of the terms of your
employment contract and shall be caused for the termination of this LETTER AGREEMENT.

school gives you the opportunity to study MASTERS DEGREE to enhance your teaching skills.

1. You will have three years of service as FULL TIME TEACHER in this institution, starting S.Y.
2019-2020 the school year which you start your Masters Degree also;

2. The St. Paul University where you are studying Master’s Degree asked sixty thousand pesos only
(Php.60, 000) for your school fees. This institution will pay the forty thousand pesos (Php.
40,000) in your school fees and you will pay the twenty thousand pesos (Php. 20,000) of your
school fees;

3. If you transfer to other institution without arriving three years of FULL TIME TEACHING
Service in this institution you are asked to pay the one hundred percent (100%) of your Masters
Degree School Fees according to the given amount by the school where you are studying.

allowed you are required to report here in school every MWF (Monday, Wednesday and Friday).
During TTH (Tuesday and Thursday) you have to work at home.

may be specifically given to you by the PRINCIPAL from time to time who shall also evaluate your
work performance and attitude every now and then as part of the school’s human resource
development program.

HOURS every day from Monday thru Friday. SIX (6) HOURS of Academic Teaching and TWO (2)

8. Your details of duty are those provided by the SCHOOL ADMINISTRATION other than those that
may be specifically given to you by the PRINCIPAL from time to time who shall also evaluate your
work performance and attitude every now and then as part of the school’s human resource
development program.

imposed from time to time and any violation thereof shall be a cause for the termination of this

 3. If you transfer to other institution without arriving three years of FULL TIME TEACHING Service in

LETTER AGREEMENT

Dear Ms. Genebeth M. Cacayan:

We take pleasure in accepting your offer to provide the GESU EUCARISTICO ACADEMY
on an EMPLOYMENT WITH A FIXED PERIOD as FULL TIME BASIC EDUCATION TEACHER from
August 24, 2020 until June 31, 2021 under the following terms and conditions.

1. Your services will be utilized on a FULL TIME EMPLOYMENT BASIS to report for EIGHT (8)
HOURS every day from Monday thru Friday. SIX (6) HOURS of Academic Teaching and TWO (2)
HOURS of documents work. However, in this time of pandemic when face to face contact is not
allowed you are required to report here in school every MWF (Monday, Wednesday and Friday).
During TTH (Tuesday and Thursday) you have to work at home.

2. Your details of duty are those provided by the SCHOOL ADMINISTRATION other than those that
may be specifically given to you by the PRINCIPAL from time to time who shall also evaluate your
work performance and attitude every now and then as part of the school’s human resource
development program.

3. You shall abide and adhere strictly to all Rules and Regulations of the school and those that may be
imposed from time to time and any violation thereof shall be a cause for the termination of this
LETTER AGREEMENT after due process.

4. That for and in consideration of your services, you will be paid the amount of five thousand and five
hundred pesos only (P 5, 500) PER MONTH with the allowance of five hundred pesos (500).

5. You shall promptly notify the ADMINISTRATION of any grievance or claim that you may have, and
anything not reported shall be deemed to have been waived and shall expire with the agreement.
 6. The attached PERSONAL DATA SHEET shall form part of this LETTER AGREEMENT and any
falsification or omission of any vital information shall be considered violation of the terms of your
employment contract and shall be caused for the termination of this LETTER AGREEMENT.
