3. Tell us about the Executor/Admin/Personal Rep. (Living you)

Fiduciary title dropdown: EXECUTOR
Choose: “I am the executor, administrator, or personal rep”

4. Mailing address: address where the bills (aka: presentments) are delivered.

5. It will auto populate and add ESTATE to the end of your ALL CAPS NAME.
Should read: ALL NAME CAPS ESTATE Check spelling. Start over if necessary.

Date Estate created/funded/probated: Month/year of 18th birthday or no further than

Choose to get online. Check spelling/info. Continue. PDF link. Print/Download.
