Dear Rico

We need more information for your Student Allowance application

Your application is currently being processed but we need some more information.

We need:

l the bank account number you want to use for payments. Please send us either a
pre-printed deposit slip with your bank account number or a bank statement with your
name and your bank, branch and account number. If it's hand-written, please ask your
bank to stamp it*
l your parents' address/es
l a statement from your parents detailing any income they receive and how much it is.
For more information, including examples of what is considered income, please refer to
our website www.studylink.govt.nz.
l proof of your parents' annual income. We will contact your parents requesting the
information or evidence we require*.

*We need to see a verified copy - do not send us original documents. A verified copy is a
copy of the original document which has been signed by someone like a lawyer, Justice of the
Peace, an approved person at an education provider, or a StudyLink or Work and Income
staff member. They must print their name, title, write that it is a true copy and then sign it.

To complete your application you still need to:

l sign the enclosed 'Student's form'

Please send any documents along with the pre-filled application. We recommend that you
send your documents to us online using www.connect.co.nz. Please remember to include
your name and client number with any documents that you send to us.

If you don't provide this information by the end of the course for which you have applied for
this Student Allowance, your application will be closed.
