1. Approved letter of request for shifting addressed to the Dean of the accepting college duly noted by the respective Dean
should be attached in this form.
• Requesting students must meet the requirements set by the department before they will be allowed to shift to the
new program.
• Certification of grades should be attached to this application for evaluation of the Department Chairperson of the
new program.
2. Release of forms will start during the release of final grades for the semester. Failure to return this form to the College
Registrar’s Office without any prior notice a week after issuance would result to disapproval of the request.
3. LAMP Scholars should secure approval from the LAMP Manager.
