INSTRUCTIONS:
Fill out the application form properly and clearly,
Email the accomplished form to your Program Chair for his advice and approval,
Upload the accomplished application form to this link, http://bit.ly/TIPM-UploadForm_RegistrarOffice for verification
and encoding of the courses by your Program In-charge,
Pay the required fees in person at the Tellering Section or through the BDO online payment (ask for your Payment Reference
Number from the Student Accounting Office),
Check your ARIS Student Portal if the changes/modifications are already reflected.

IMPORTANT:
Registration in any course without all the prerequisites, correct sequence or authorized load will not be given credit regardless of the
grade/s obtained.
All enrollment and/or related changes will only become valid if payment of the corresponding fees is made within the day.
