Dear Participants,

This study aims to determine the home visitation program its effectiveness to pupils
academic achievement . As the respondents of the study you are pleased to answer the
given questions to you. Part I will focus on the demographic profile of the
respondents. Part II contains of questions based on your observation of your children
in home visitation program.
