1- INTRODUCTION
This instruction outlines the general requirements of Southern Area Well Completion Operation
Department (SAWCOD) Risk Assessment and Management set out in SMS Element # 2, and the Job Safety
Analysis (JSA) process set out in SMS Element # 6 of the Saudi Aramco Safety Management System.
2- OBJECTIVES
2.1. To identify all potential hazards for a specific job.
2.2. To eliminate or manage hazards prior to beginning a job.
2.3. To form the basis for the development of safe operating procedures.
2.4. To help plan to perform a job safely.
2.5. To assist in training employees in safe job procedures.
3- GENERAL REQUIREMENTS
3.1. This instruction applies to all divisions and to contractors performing jobs at SAWCOD.
3.2. The need to perform a risk assessment shall be considered at the planning stage and shall
be prepared for the major jobs, accordingly a JSA has to be developed for the identified
critical operations which involve a significant risk, which are not adequately controlled by
existing control measures.
3.3. A job inventory list to be developed for the activities that requiring JSA.
3.4. The risk assessment shall be conducted by a multidisciplinary team from operations,
engineering, HSE specialist, etc. led by an individual familiar with the methodology. This
team shall have considerable knowledge and experience of the department operations.
3.5. Biased on the risk assessment result, the assigned team will list the critical activity with
reference to hazards set forth in risk register.
3.6. JSAs shall be developed for the activity listed in job inventory, where possible, involve
personnel actually doing the job.
3.7. Completed JSA’s shall be attached and or referenced to any WAP issued for the job. Be
made available for review/reference by affected personnel and the results shall be
communicated to personnel performing the job via safety meetings or “one on one
instructions”.
3.8. Unit Heads, or their designee must review the completed JSA’s with all personnel (Saudi
Aramco or Contractor) involved in carrying out the job.
3.9. Where there has been an accident or near misses, a JSA shall be performed to prevent
recurrence of similar incidents.
3.10. In a job for which a JSA has been prepared previously, the WAP issuer and receiver shall
both make sure that workers review the JSA before a work authorization permit issued.
3.11. If an injury occurs on a specific job, the JSA shall be reviewed immediately to determine
whether changes are needed in the job procedure.
3.12. If an incident has resulted from an employee’s failure to follow the JSA, this JSA shall be
discussed with all employees performing the job.
3.13. Any time a JSA is revised, training in the new job methods, procedures, or protective
measures shall be provided to all employees affected by the changes.
3.14. A JSA can be used to train new employees on the steps and job hazards
5- RESPONSIBILITIES
5.1. The Department Manager:
5.1.1 Ensures that necessary resources (i.e., personal, financial, equipment and
material) to perform effective risks assessment and evaluations are allocated
5.1.2 Approve high and medium risk activities to be continued with interim precautionary
measures within the allowable duration.
5.1.3 Ensure that appropriate resources, training, and level of authority are provided to
meet the JSA requirements
5.1.4 Consistently enforce and promote the use of JSA wherever applicable.
5.1.5 Designate JSA Coordinator.
5.2.1. Nominate and form multidisciplinary team for hazard identification and
qualitative risk assessment development.
5.2.2. Perform annual review of the qualitative risk assessment process.
5.2.3. Perform annual review and update of the risk register or when deemed necessary.
5.2.4. Communicate high and medium risk activities to department manager with
interim precautionary measures.
5.2.5. Monitor and communicate KPIs status and pending risk register actions during the
quarterly Safe Operation Committee (SOC).
5.2.6. Share the final risk assessment result of a newly introduced technology, service or
equipment, etc. with the operation facility upon approval.
5.3.1. Perform risk assessment to identify the associated hazards in the department
operations.
5.3.2. Document the identified hazard in a risk register.
5.3.3. Develop job inventory for the identified critical operations in reference to the
developed risk register.
5.3.4. Identify and document the existing protections (safeguards) for the identified
hazards in the risk register.
5.3.5. Identify additional measures if the identified hazard is not tolerable with the
existing protection.
5.4.8. Approve JSA’s for extremely hazardous jobs which are brought to his attention by
the Unit Head or the Safety Advisor
5.6.1. Review and apply the protection measure of the risk register.
5.6.2. Implement approved job safety analysis (JSA).
5.6.3. Report to direct supervisor and through EHSM a newly recognized hazard.
5.6.4. Ensure that all involved personnel are fully aware of associated hazards and their
control measure.
5.6.5. Involved personnel shall participate and contribute in hazard assessment meetings
as appropriate
5.6.6. Ensure the procedures and safety precautions outlined in JSA are followed at all
times
5.6.7. Assist in conducting JSA as assigned
5.6.8. Maintain up-to-date copies of all JSA’s in his office and make them accessible to
employees
5.6.9. Review JSA’s with affected employees
5.7.1. Assist Unit Heads in identifying training resources, as required by the Unit Heads
(JSA training should be the responsibility of Unit Heads)
5.9.1. Fully comply with the contract, including identifying and addressing all potential
safety, health and environmental hazards.
5.9.2. Apply the protection measure of the risk register & job safety analysis as a minimum
requirement.
5.9.3. Develop risk assessment for each specific activity and apply protection measures.
