(COMPLAINT AFFIDAVIT)

Ako, si ALVIN CAMITAN MARASIGAN, 35 na taong gulang, Pilipino, Factory

worker at nakatira sa Barangay Banga 2nd, Plaridel, Bulacan, na matapos na
makapa-numpa ng na-ayon sa batas ay malaya at kusang loob na nagsasalaysay ng
mga sumusunod:

• Aking inirereklamo si JAYKARL MANLAPAZ @JAKAR ay nasa hustong

gulang, Pilipino, at nakatira at residente ng sa Barangay Banga 2nd ,
Plaridel, Bulacan dahil sa ginawa niyang walang awang pagpatay sa aking
pamangkin na si EDUARD JOHN MARASIGAN sa pamamagitan ng pag
baril nito noong HUNYO 13, 2021 alas 08:55 ng gabi sa Lipana St. Brgy.
Parulan, Plaridel, Bulacan;

• Na noong bandang alas 8:25 ng gabi, ang mga inirereklamong si JAYKARL

MANLAPAZ @JAKAR ay kasama ko na nakikipag-inuman sa biktimang si
EDUARD JOHN MARASIGAN. Kasama pa ang iba kong kaibagan na sina
CHRISTIAN MONTERO @APODADA at EDU MARCELO @TEBAK. Alam
namin na may matagal na silang may alitan dalawa dahil sa lupa. Habang
kami ay nag iinuman si EDUARD JOHN MARASIGAN ay masayang nag
kwe-kwento. habang si JAYKARL MANLAPAZ @JAKAR ay biglang nag
paalam at siya raw ay iihi lamang, at hindi na ito bumalik. At si EDUARD
JOHN MARASIGAN ay nag paalam na uuwi na dahil siya raw ay hinahanap
na. At kami ay biglang nagulat sa umaalingaw-ngaw na tatlong putok ng
baril.
• At ako ay biglang tumakbo at hinanap kung saan banda ang putok ng baril.
 At nakita ko si JAYKARL MANLAPAZ @JAKAR ay tumatakbo palayo
habang ang aking pamangkin na si EDUARD JOHN MARASIGAN ay naka
handusay at naliligo sa sariling dugo at wala nang buhay.

• Ang baril na ginamit sa pag paslang ng inirereklamo na si JAYKARL

MANLAPAZ @JAKAR ay aming nakuha lugar ng pangyayari at aming
nilitratuhan habang ito ay may bahid pa ng dugo. Lakip dito bilang
ANNEXES “A”, “A-1”, “A-2”, “A-3” ang mga litrato ng nasabing baril na may
dugo na ginamit sa pagpatay sa biktima;

• Ang kopya ng (1) Salaysay sa Pagsasampa ng Reklamo ni ALVIN CAMITAN

MARASIGAN, (2) Salaysay ng Pagtestigo ni CHRISTIAN MONTERO
@APODADA, at (3) Salaysay ng Pagtestigo ni EDU MARCELO @TEBAK ay
nilalakip dito bilang ANNEXES “_”, “_”, “_”, “_”, “_”, “_”;

Na ginawa ko po ang reklamo na ito upang patotohanan ang lahat na dito ay

nasusulat at upang ipabatid sa mga kinauukulan na karapat-dapat sampahan ng
kasong MURDER ang mga inirereklamo na sina JAYKARL MANLAPAZ @JAKAR
dahil sa ginawa nilang walang awang pagpatay kay EDUARD JOHN MARASIGAN

SA KATUNAYAN NG LAHAT na ito, ako ay lumagda ngayong ika-14 ng JULY

2021 dito sa Lungsod ng Malolos, Bulacan.

AFFIDAVIT OF ARRESTING OFFICER

• I, PSSG. JOHN MICHAEL TOMINIO, of legal age, single, regular member of the
Philippine National Police based at Calumpit Police Station Calumpit,
Bulacan after having been sworn to in accordance with the law do hereby
depose and say to wit;
• That day 09:30 o’clock in the evening, AUGUST 18, 2022, I was patrolling
with PCpl. John Michael Magsakay in the road of Barangay Balite, Calumpit
Bulacan. We received cellular phone call from our desk officer to go in alleged
incident in Purok 5 of Barangay Balite, Calumpit Bulacan.
• On or about 09:32 o'clock in the evening of the same date, we arrived in crime
scene and we found dead body, blood, and a gun. As a first responders we
secure the crime scene to preserve and protect the evidences because of
overcrowding and report immediately in our desk officer to call SOCO team.

• During the initial investigating of SOCO TEAM, I and PCpl. John Michael
Magsakay conduct an interview of witnesses to get some information.
• On or about 09:35 in the evening of the same date, the team have received an
information that person of interest is in his house in Barangay Balite
Calumpit, Bulacan.
• On or about 09:40 o'clock in the evening of AUGUST 18, 2022, I was with my
team to conduct Hot Pursuit. Because we have an information that suspect is
in his home.
• The Calumpit Police Station has arrived in the house of suspect Alvin
Camitan @KUYANG. And we noticed the Gate is locked, so we need to open
by using force. After we broke the Gate we noticed a man is crying in the
kitchen and his hands and t-shirt have splatters of blood. He surrender
and appraised him of his constitutional rights but he opted to remain silent.
The team together with the arrested suspect immediately proceeds to
Calumpit Police Station for the conduct of marking, inventory and
documentation of confiscated pieces of evidence. The arrested person was
later identified as ALVIN CAMITAN @KUYANG, 40 years old, married, jobless
and a resident of Barangay Balite Calumpit, Bulacan.

• On or about 10:15 in the evening of the same date, The inventory of the pieces
of evidence in crime scene and the suspect were done in the presence of the
arrested person (ALVIN CAMITAN @KUYANG) as witnessed by Lloyd anciro,
Chairman of Brgy. Balite Calumpit, Bulacan and Atty. Ernesto Casidsid a
Representative from the Department of Justice (DOJ). No representative from
the media witnessed the inventory despite earnest efforts made in requesting
their presence.

• I am executing this affidavit freely and voluntarily as I attest to the

truthfulness and veracity of the above mentioned facts.
