DATA PRIVACY CONSENT FORM

Thy Covenant Montessori School, Inc. (the “School”) is bound by the Data Privacy Act of 2012 (the “Act”).  It
is committed to respecting each student’s personal privacy while ensuring its ability to fully carry out its
responsibilities. This Privacy Policy sets out how the School manages personal information provided to or
collected by it, who uses and how the information is used, how the information is shared, how long the
information is retained, how a data breach is handled and what the student’s rights are with respect to
his/her personal information. The School may, from time to time and at its sole discretion, review and
update this Policy to take account of new laws and technology, changes to the School’s operations and
practices and to make sure it remains appropriate to the changing School environment.  The School will
provide a notice of any such modification, which shall be effective immediately upon posting or publication.

Information Collected
TCMS collects, stores, and processes personal data from its current and past students, starting with the
information collected during enrollment throughout the whole course of study at the school. This will
include:
∙  Contact information, such as, names, addresses, telephone numbers, email addresses and other contact details

∙  Personal information, such as date and place of birth, nationality, religion, civil status, student ID, government-issued
IDs, etc.
∙  Family background, including information on parents, guardians, siblings, related TCMS alumnae, etc.

∙  Photographic and biometric data, such as, photos, CCTV videos, fingerprints, handwriting and signature specimens

∙  Student's school works during classroom and school activities

∙  Health records, psychological evaluation results, disciplinary records, and physical fitness information

∙  Student Cumulative Guidance Folder, which includes interviews, entrance exam results, guidance assessments,
special needs, exclusions/behavioral information, etc.
∙  Permanent Student Academic Records, including transcripts and the academic history of the student in TCMS.

∙  Student extra-curricular activities

∙  Financial and billing information

Use of Information
The collected personal data is used solely for the following purposes:
∙  Processing of admission application (and to confirm the identity of students and their parents)

∙  Verifying authenticity of student records and documents

∙  Processing of enrollment and registration

∙  Supporting student learning, and validating students’ program of study based on curriculum requirements, and
other activities and experiences forming part of the student’s formation and education
∙  Supporting the student’s well-being and providing medical services and guidance counselling

∙  Monitoring and reporting on student progress; processing of evaluations, exam results, and grades

∙  Monitoring and ensuring the safety of all students within TCMS campus and TCMS' partner school TIS (Treston
International College) campus for SHS students.
∙  Processing and generating statements of accounts

∙  Processing of application for completion and graduation

∙  Documentation of students’ data

∙  For accreditation, professional development of teachers and staff, and research, e.g., evaluation studies by the
research desk, action research by teachers, etc.
∙  Posting or displaying of academic and non-academic achievements (e.g. general average and ranking during PPTC)
within the TCMS’/TIS' premises)
•Posting in all school programs (co-curricular, extra-curricular, monthly programs e.g. buwan ng wika etc)
∙  Marketing and promoting TCMS, its students, and other academic and non-academic student and/or school
activities inside and outside the campus
∙  Providing Library services, running an outreach program, family council purposes, practicums.

Information Sharing
Personal data under the custody of TCMS shall be disclosed only to authorized recipients of such data. Otherwise, we
will share your personal data with third parties other than your parents and/or guardian on record, only with your
consent, or when required or permitted by our policies and applicable law, such as with:
∙  Regulatory authorities, courts, and government agencies, e.g., Department of Education, etc.

∙  The Private Education Assistance Committee (PEAC) has been contracted by the DepEd to administer the ESC and
TSS programs. PEAC is the trustee of the Fund for Assistance to Private Education (FAPE), a perpetual fund created to
provide assistance to private education in the country.
∙  Service Providers who perform services for us & help us support your learning, monitor & report on your progress

∙  Business partners and other academic linkages who provide internships and job opportunities to our graduates.

Security
The School’s staff are required to respect the confidentiality of your personal information and the privacy of
individuals. The School has in place steps to protect the personal information it holds from misuse, interference and
loss, unauthorized access, modification or disclosure by use of various methods including locked storage of paper
records and password access rights to computerized records.

Retention of Information
The School will retain your personal information indefinitely for historical and statistical purposes.  In cases where a
retention period is required by law, all records after such period will be duly and securely disposed of. 

Your rights
You have the right to be informed, object to processing, access and rectify, suspend or withdraw your personal data,
including, any such information held by third parties, with whom TCMS have a data sharing agreement; and be
indemnified in case of damages pursuant to the provisions of the DPA and the DPA-IRR.

If you have a concern or complaint about the way we are collecting or using your personal data, you should raise your
concern with us in the first instance.

TERMS OF AGREEMENT
On behalf of the applicant/student, I have read the Policy and the Terms herein written, and agree to be bound by
them. I understand that the Policy and Terms refer to the personal data of my child/ward.
By signing below, I agree with the following:
● Republic Act No. 10173, or the Data Privacy Act of 2012, and other relevant Philippine laws apply to the
collection and processing of my child’s personal data.

● I understand that by applying for admission/registering as a student of TCMS, I am allowing it to collect, use
and process my child’s personal data where a legitimate educational or institutional interest exists in its
determination, as enumerated in the Policy and other like circumstances.
● By providing personal information to TCMS, I am confirming that the data is true and correct. I understand
that TCMS reserves the right to revise any decision made on the basis of the information I provide should the
information be found to be untrue or incorrect.
● By entering into this Agreement, I am not relying upon any oral or written representations or statements
made by TCMS other than what is set forth in this Agreement.
● My agreement to the Policy and Terms is among the conditions to my child’s admission into TCMS.
● Any issue that may arise in connection with the processing of my personal information will be settled
amicably with TCMS before resorting to the appropriate arbitration or court proceedings within Philippine
jurisdiction.
