Integration Session
A 22 year old Man was rushed to the emergency room due to injury in the glabellar area. He was driving
a motorcycle without a helmet and under the influence of alcohol when he hit a jeepney and fell head
first on to the pavement. The patient claims to have had epistaxis and doubling of vision on upward
gaze, he however denies any loss of consciousness or headache. Upon inspection you noted a laceration
on the forehead and the philtrum. There is also gross deformity in the nasal dorsum and the glabella.
Upon palpation you noted crepitation in the area. The patient also noted widening of his intercanthal
distance.
1. Upon seeing the patient, what will be your initial management for the patient at the ER?
2. What additional information would you like to illicit from the patient?
3. What pertinent physical examination findings will you look for in this trauma patient?
4. What diagnostic tests will you request?
5. Given this imaging study, Provide your primary impression.
6. What is the appropriate treatment for this patient?
A 2 year old male consulted at the ENT-HNS OPD due to cleft lip. He was born full-term to a then 25-
year-old G1P0 via normal, spontaneous delivery without complications aside from noted cleft lip. His
mother had bilateral cleft lip.
1. What other history and physical examination findings are important to elicit in patients with
cleft?
2. What additional diagnostic tests will you request to determine if the patient is fit to undergo
surgery?
3. What appropriate intervention should be done for the patient prior to surgery?
4. Elaborate appropriate management required for our patient to be done post operatively as he
grows to adulthood.
