PERMIT TO WORK

PROCEDURE

1. PURPOSE AND SCOPE

This Permi to work (PTW) is designed to ensure that all elements of risk in work activities are considered in the
planning, approval, preparation, execution and completion of the strictly activities in Hayat area.

It is aims to create a safe working condition by providing management control over the work activities. The PTW
procedure serves as a tool to:

 Prevent unexpected incidents/accidents and control risks;


 Document safety measure taken and appropriate approval signatures;
 Communicate information related to the work activities to the responsible persons;

All PTW issued for the work activities shall be documented and the records maintained for the duration of takes
place. HSE team will save this docs in 3 months.

This management plan applies to all project personnel and Subcontractors working at Hayat site and all
employee of Hayat Kimya Vietnam .

2. DEFINITIONS

 PTW: a written document that authorizes certain people to carry out specific work at a certain time and with
stated precaustions to complete the job safely.
 Safety requirement: safety measures to be observed in the acvitities

3. RESPONSIBILITY

The following roles and responsibilities are assigned to all personnel involved in the planning,
approval, preparation, execution and completion of work at the construction site.

3.1 Hayat Department Manager


The each department Manager is responsible for:
 Ensure the setting up of Permit to Work system;
 Review and sign off PTW;
 Able to revise and sign PTW; and
 Assign EHS officer for review PTW of each section and sub-contractors;
 Open the PTW;
 Close the PTW.

3.2 Hayat HSE department


The Hayat HSE department will:
 Set up and run a Permit to Work system that will be used during construction, pre-commissioning and
commissioning and all maintenance activities in Hayat Kimya Vietnam area.

PROCEDURE
 Verify that the Hayat or Subcontractors work are carried out strictly in accordance with the approved
Work Method Statement and associated Permit to Work or JSA;
 Review the proposed activities of PTW;
 Ensure that the Hayat or Subcontractors obtain any necessary Permit to Work prior to starting any
relevant task;
 Confirmation that any Permit to Work required for the work have been obtained; and

3.3 Sub-contractor Site Manager/Section Manager and HSE Officer


The Sub-contractor Site Manager and HSE Officer will:
 Ensure compliance with PTW in arrangement for specific working condition;
 Check the implementation of PTW every day;
 Sign off under applicant name; and
 Return the PTW for closure upon expiry date.

3.4 Sub-contractor Site Supervisor


The Sub-contractor Site Supervisor will:
 Prepare the PTW and necessary documents;
 Check the implementation of PTW every day;
 Sign off under applicant name; and
 Return the PTW for closure upon expiry date.

3.5 All Hayat employee and Sub-contractor


The Hayat employee and Sub-contractor will ensure:
 The personnel executing the work have the necessary skill to carry out the work activity;
 The right equipment are used for the work to be done;
 Safety precautions are affected to and the work is carried out in accordance with the conditions stated in
the PTW.
 Maintain good housekeepting during execution of the work and leave the work site tidy upon completion
of the work.

4. TYPES OF PERMIT TO WORK

Before conducting work that involves entry into confined space, work on energized systems, ground disturbance
in locations where buried hazards may exist, or hot work, a permit will be developed by the Sub-constructor Site
Manager/HSE officer and submit to Hayat department Manager who requested that activities to review and sign
off in a form including information of:

 Defines scope of work;
 Identifies hazards and assesses risk (risk assessment matrix);
 Establishes control measures to eliminate or mitigate hazards;
 Specifies the PPE to be used;
 Links the work to other associated work permits or simultaneous operations;
 Is authorized by the responsible person(s) within HAYAT’s management team;
 Communicates above information to all involved in the work; and
 Ensures adequate control over the return to normal operations.
PTW form is attached in Appendix.

Issue and Closure

The PTW will be issued for the duration of one week. New PTW will be issued when the current PTW expired.
PTW number will be recored in a log.
When issued, the original copy must be kept at the work site. One copy will be kept at related Section Office and
one copy will be kept at the HAYAT HSE Department.
PTW will be closed at the completion of the work.

Document to be attached

Supporting documents including risk assessment and method statement. Specific documents need to be
attached based on specific works (for example lifting plan etc).

5. REQUIREMENTS FOR EXECUTION, SUSPENSION AND COMPLETION

5.1 Work permit execution


Work can start when the permit has been approved and the site has been checked and cleared. An approved
work permit will be kept at the site and review by the execution team prior to start of work.
5.2 Work permit suspension
A work permit can be canceled or suspended at any time if:
 There is an emergency affecting the work site;
 The requirements of the work permit are not fulfilled;
 A dangerous situation arises.
5.3 Work permit completion
All personnel involved in the work upon completion of work shall inspect the site to ensure that:
 The work has been completed safety and fullly;
 The equipment has been rendered un-operable and reactivated in a safe mode and the work site
cleaned up; and

 Return the PTW to the Vestas HSE Department for closure.
