Whereas, Bayawan National High School will celebrate its 26 th Founding Anniversary on October 16 – 17,
2019.
Whereas, activities have been lined up in order to make the celebration more relevant and exhilarating. Such
activities are also a culminating activities and presentation of the students’ output in various
subjects in their respective strand/department.
Whereas, the amount of One hundred pesos will be collected from the parents to defray expenses such
costume, Mr Ambassador & Miss Ambassadress 2019, Eco-wear and other expenses relevant
thereto.
Whereas, the said contribution is an expression of support by the parents to the school to make the celebration
meaning and successful,
Resolved as it is hereby resolved, proper use of money shall be observed. Report of expenses shall
be presented to the parents during meetings/conferences.
Resolved further, a copy of this resolution shall be forwarded to the Division for proper action.
Now therefore, on motion of Laarni Trayvilla duly seconded by Marlou L. Gayanelo, both Board of
Directors of the PTA that parents should contribute the amount of One Hundred pesos in support for the school
as it celebrates its 26th Founding Anniversary..

Whereas, source/s of income are the membership fee/registration fee, contribution, fines and other
related income of the association,
Whereas, for the purpose of safekeeping of the monetary income of the association, there is a need of
opening an account to the nearest reputable bank and all income of the association shall be
deposited in this bank.
Resolve as it is hereby resolved, that General Parents-Teachers Associations, GPTA shall open
an account at BDO Unibank, Inc. – Bayawan Branch and shall bear the name, GENERAL PARENTS-
TEACHERS ASSOCIATION OF BAYAWAN NATIONAL HIGH SCHOOL – SENIOR HIGH SCHOOL, (GPTA-
BNHSSHS),
Resolved further, that withdrawals of money shall be made only through the school Principal
and the duly elected president and/or treasurer of the association.
Resolved furthermore, that the association shall submit the list of officers or the association’s
organizational set up every year for the purpose of updating and for the change of signatories
specifically for withdrawals of money. The current Principal is DR. CHERRYL MAY A. HONGCUAY, current
President is MRS. MA. MELBA D. FISHER and Treasurer is MRS. LAARNI TRAYVILLA,
Finally resolved, that a copy of this resolution will be forwarded to BDO Unibank, Inc. Bayawan
Branch, National Highway, Bayawan City, Negros Oriental.
Now therefore, on motion of Analyn I. Ongue duly seconded by Marlou L. Gayanelo both Board
of Directors of the association that the General Parents-Teachers Association (GPTA) will open a bank
account at BDO Unibank, Inc. Bayawan Branch, National Highway, Bayawan City, Negros Oriental for the
safekeeping of the monetary income of the association.

A RESOLUTION REQUESTING THE RELEASE OF FORTY THOUSAND PESOS (P40,000.00) FROM THE
PARENTS TEACHER ASSOCIATION (PTA) FUND TO DEFRAY THE EXPENSES FOR THE ACQUISITION
CAMERA, MICROPHONE AND SOUND MIXER FOR SCHOOL USE.
Whereas, the said equipment is vital to the operation of the school especially during activities which
require documentation,
Whereas, school has no budget to fund for the acquisition of the said equipment,
Resolved as it is hereby resolved that the Parent Teacher Association of Bayawan National High
School Senior High School Department through its special meeting held on October 15, 2020 at Bayawan
National High School Senior High School Department campus, Villareal Bayawan City Negros Oriental will
shoulder the expenses for the acquisition of the aforementioned equipment subject for usual
accounting rules and regulations,
Resolved further that said equipment will serve as donations of parents in support for all the
activities of the school,
Resolved furthermore that copies of the resolution shall be forwarded to the servicing bank in
support for its withdrawal.

A RESOLUTION REQUESTING THE RELEASE OF TWENTY THREE THOUSAND PESOS TO DEFRAY THE
EXPENSES FOR THE PURCHASE ADDITIONAL PARAPHERALIAS FOR SOUND SYSTEM
Whereas, the PTA has already donated partial paraphernalia for the sound system
Whereas, the said paraphernalia is still lacking and additional materials need to be purchased for the
completion of the said sound system
Whereas, the following are the needed materials for the sound system
2 units speaker 15,000.00
2 units Speaker stand 3,600.00
70 m Speaker wire #16 1,050.00
4 pcs Speaker connector 1,440.00
Misc. 2,000.00
Total 23,090.00
Resolved as it is hereby resolved that the Parent Teacher Association of Bayawan National High
School Senior High School Department through its special meeting held on August 8, 2021 at Bayawan
National High School Senior High School Department campus, Villareal Bayawan City Negros Oriental will
shoulder the said expenses for the purchase of the aforementioned equipment subject for usual
accounting rules and regulations,
Resolved further that said equipment will serve as donations of parents in support for all the
activities of the school,
Resolved furthermore that copies of the resolution shall be forwarded to the servicing bank in
support for its withdrawal.
Now therefore on motion of Analyn I. Ongue duly seconded by Laarni Trayvilla both board of
directors of the association that the said equipment will be purchased through PTA Fund and be
donated to Bayawan National High School - Senior High School.

A RESOLUTION REQUESTING THE RELEASE OF SEVEN THOUSAND PESOS TO DEFRAY EXPENSES FOR
THE INSTALLATION OF THE NEW SCHOOL HEAD AND TURN OVER CEREMONIES IN BAYAWAN
NATIONAL HIGH SCHOOL – SENIOR HIGH SCHOOL DEPARTMENT
Whereas, Bayawan National High School – Senior High School Department will have a designated a new
school head as per division directives
Whereas, an installation and turn over ceremonies will be held on January 7, 2022 at 2 o’ clock in the
afternoon at FBS Room of said school,
Whereas, as token of gratitude to the outgoing principal and welcome celebration of the newly-installed
school head of the school, the organization will donate one (1) piece whole pork lechon and
cash for the miscellaneous expenses for the program
Now therefore on motion of Analyn I. Ongue duly seconded by Laarni Trayvilla both board of
directors of the association that an amount of SEVEN THOUSAND PESOS will be withdrawn from the
organization’s fund to purchase the above mentioned donation.

Whereas, the Parent Teachers Association (PTA) is a duly recognized parent organization
in school whose function serves as support group and as partner of the school in
terms of students’ welfare and development as stipulated in Deped Order No. 54
s. 2009;
Whereas, effective school year 2020 – 2021 and 2021 – 2022, the department of
Education has adopted a NO FACE TO FACE learning modality to
continuously deliver the learning process amidst covid – 19 pandemic;
Whereas, recently the Inter Agency Task Force (IATF) has categorized the entire
province of Negros Oriental under Alert Level II in terms of community
quarantine status in which Villareal, Bayawan City where Bayawan National
High School – Senior High School is located,
Whereas, Deped-DOH JMC No. 1 s. 2021 allows limited face to face of classes under
Level I and II classification in terms of community quarantine status,
Whereas, Bayawan National High School – Senior High School will be participating the
limited face to face of classes as required by the Department of Education,
Resolved furthermore, that the school will strictly adhere on IATF protocols
such as wearing of face mask/shield, social distancing and other precautionary measures
to ensure the safety of our children.
