Step 1 Registrar: a) Issuance of application for enrolment, prospectus, ID number and Evaluation of subjects regular and
transferee b) Submission of Credentials

SINDANGAN ZAMBOANGA DEL NORTE has duly recognized this ENROLMENT SLIP as a contract between me and
the SOUTHERN MINDANAO COLLEGES and as such this serves as my record in this school; that I agree not to
drop or change any subject appearing on this slip without due notice and approval of the Dean and the
Registrar. Otherwise, I bind myself to pay for any subject dropped or changed (without the notice or knowledge
of the Dean and Reagistrar's Office): that if I fail to submit my admission credentials within the required period
of time, I shall be considered as unofficially enrolled and thus all subjects I am taking will not be credited: that I
am agreeable to the rate of tuition and other school fees that are charged in the course I am enrolled that I am
not a member of a radical organization or group of persons who uphold and teach doctrines against our
government or defend or teach violence, personal assault or assasination for the success and performance of
man's ideas. I have read the rules and regulations of the SMC as specified in the Student Handbook, and that, I
hereby promise to abide by the school policies immediately upon my admission.

I am fully aware that this contractual relationship is renewable every semester. The college
reserves the right to renew or not the contract. Any infringement I have made to the rules and regulations of
the school is a ground for recession of this contract even before the expiration of the period.
