SOP On Medical Checkup
1. OBJECTIVE
2. SCOPE
This procedure is applicable for all employee i.e. Factory Staff /Corporate Staff & Contract workers
for pre joining and working at XXXXXXXX.

4. PROCEDURE
4.1. Each employee shall undergo Medical check-up at the time of joining the organization
and periodically thereafter as per requirements.
4.2.5. For those persons who are supposed to work in material dispensing (Warehouse),
manufacturing, filling and printing lines, medical checkup needs to be completed
within 10 working days.
4.2.6. For all other persons, medical checkups need to be completed within 3 months from
the day of joining.
4.2.7. Registered Medical Practitioner shall review test report of employee & fitness for job
shall be decided based on satisfactory medical examination report. Clearance for
joining shall be given based on satisfactory medical report / fitness certificate.
4.2.8. The details of report of each employee shall be recorded in Annexure – I by
Registered Medical Practitioner & handover to HR Department.
4.3.1. Periodic examination shall perform once in year with 30 days grace period.
4.3.2. Periodic Medical check-ups as follows:
– Physical test
– Complete blood count (CBC)
– Erythrocyte sedimentation rate (ESR)
– Eye Check-up
– Random Blood Sugar (RBS)
– Urine examination
– Creatinine (For Kidney)
– SGPT (Serum Glutamic Pyruvic Transaminase)
4.3.3. In addition to above, Pulmonary Function test (PFT) shall be conducted for those
persons who are supposed to work in “hazardous process” or “dangerous
operations” as defined in the Factories Act, 1948/Gujarat Factory Rules as amended
from time to time.
4.3.4. Audiometry Tests shall be conducted every 6 months with 30 days grace period for
those persons who are supposed to work in areas having occupational noise level 90
dB(A) and above.
4.3.5. Employees working in Inspection/Maintenance/Forklift & Company Drivers shall
undergo eye examination in addition to regular medical check-up. Such examination
shall be conducted every 6 months with 30 days grace period. Eye Examination
report shall be furnished as per Annexure – II.
4.3.6. In case of any abnormality or disease found during Periodic examination, then
necessary action shall be taken under guidance of Registered Medical Practitioner.
The details of report of each employee shall be recorded by approved Registered
Medical Practitioner in Annexure – I. On the basis of satisfactory medical reports
employee shall be given clearance for performing the assigned job.
4.3.7. In case of an employee resuming duties post medical leave, each employee shall
report to HR Department. The employee shall furnish medical fitness certificate from
approved Registered Medical Practitioner for fitness to resume work, in case he/she
has availed leave for 7 days or more. In case of any ambiguity, the company has the
right to take a second opinion from the company appointed medical practitioner.
4.3.8. HR Department shall maintain fitness records as per Annexure – III.

4.3.9. List of Employee Medical Check-up Calendar shall be maintained as per Annexure –
IV.
