Essential Questions
 What are the different School Forms that teachers in DepEd prepare and submit?
 Why do we need to prepare and submit the various school forms?
Lesson Overview
 Schools Under Department of Education (DepEd) and any private schools adopts
various school forms for various purposes. Teachers are to prepare and submit
these forms to concerned offices and School Head as part of their job functions.
 DepEd forms include the following but not limited to:

 School Form 1 (SF 1) School Register


 School Form 2 (SF2) Daily Attendance Report of Learners
 School Form 3 (SF3) Books Issued and Returned
 School Form 4 (SF4) Monthly Learner's Movement and Attendance
 School Form 5 (SF 5) Report on Promotion & Level of Proficiency
 School Form 6 (SF 6) Summarized Report on Promotion and Level of Proficiency
 School Form 7 (SF7) School Personnel Assignment List and Basic Profile

School Forms Brief Description and Use


School Form 1 (SF 1) School Register A school register also known as an attendance
register, is an official list of students who are
present at the institution. can also be called as list
of student profile which includes names, birthdate,
religion, address, age, name of parents and the
contact numbers and etc.
School Form 2 (SF2) Daily Attendance Report It is a form that the prescribed period of time when
of Learners schools offer daily instruction. It covers10 months
of regular schooling starting month of June
(Beginning of School Year) up to the month of
March (End of School Year) of the following year. It
is records of how often someone has been present
in school.
School Form 3 (SF3) Books Issued and A form that keeps records when the books was

returned also visible in this form.
School Form 4 (SF4) Monthly Learner's It is a form that highlights the summary of
Movement and Attendance attendance of the students. It also includes the
number of dropped out, transferred in and out

School Form 5 (SF5) Report on Promotion and It is a form that highlights the student’s status such
Level of Proficiency as average grade, incomplete subjects taken if
ever, and the action taken if the student was
promoted, irregular, or retained. It also shows the
level of proficiency of students if they are
beginners, developing, approaching, proficient, or
advanced.
School Form 6 (SF6) Summarized Report on It is a summary form taken from SF5. SF6
Promotion and Level of Proficiency highlights the number of students being promoted,
irregular, or retained. Also the number of students
who are beginners, developing, approaching,
proficient, or advanced.
School Form 7 (SF7) School Personnel From the title itself, SF7 is a form that shows the
Assignment List and Basic Profile basic profile of the school personnel. It includes
designations/positions, educational background,
subject being taught, and their respective
programs.
E-Class Record A class record is a document or compilation of
documents used by teachers to record enrollment,
attendance and grade information of students. The
class record book may be electronic or manual and
is created, updated and maintained in accordance
with District and State requirements.
