The Form of Application for Membership

Sir,
I , Ms Ambiyabee Wahid Patel an indian inhabitant of Mumbai
Presently residing at tenement No. 372/2986,motilal nagar no 2, M.G road
,goregaon west, Mumbai-400104, (PAN No. DQWPP9776B, AADHAAR No.
699218039073)

MR. Wahid Yasin miya Patel was a member of the Society holding share bearing
serial No.__________for room No __________in the Society.
MR. Wahid Yasin miya Patel the deceased member of the society, died on
03/12/1993 , the Xerox copy of the Death certificate is enclosed here within.
The late MR. Wahid Yasin miya Patel the Deceased member of the society
had not Nominated any Person /legal Heir under Rule 25 of the Maharashtra Co-
operative Society Rules , 1961.
Being only legal heir I am entitled to make an application for membership of
the society and for the transfer of shares and interest of the deceased member in
the Capital/property of the society to my name.
I have executed the indemnity Bond in favor of the society indemnifying it
against any claim made at any subsequent time by other legal heir to the shares and
interest of the deceased member in the capital/property of the society . The said
indemnity Bond is enclosed herewith .

I remit herewith an amount of Rs. 100/- as entrance fee my particulars for
the purpose of consideration of my application for membership of the society are
as under :-

Occupation : House wife
Monthly income : Nil
Residential address: 372/2986,motilal nagar no 2,
M.G road ,goregaon west,
Mumbai-400104.
I enclose here with the undertaking and declaration in the prescribed form ,
in respect of transfer of tenement to my name under section 269 AB Of income
tax act.
I undertake to discharge the present and future liabilities of the society.
I have understood the byelaws of the society and undertake to abide by a
same and any modifications that the registering Authority may make in them.
I , Request you to please admit me as the member of the society and transfer
the shares and interest of the deceased member in the capital/property of the
society to my name. The share certificate held by the deceased member is
enclosed herewith.
I undertake to use said room premises for the purpose for which it was
allotted to or acquired by the deceased member and that any change of user will be
made with prior approval of the society , the undertaking in the prescribed form to
that effect is enclosed(appendix-3)
