PRIZE ACCEPTANCE FORM AND RELEASE FORM

IMBRT Brand Name Contest (Page 1 of 2)

The undersigned hereby acknowlegde and accepts the following prize awarded in conjunction with
the IMBRT Brand Name contest:

IMBRT Merchandise and One (1) Year Iskandar Rapid Transit Travel Pass.

(hereinafter referred to as the “Prize”), as set forth in the IMBRT BRAND NAME CONTEST Terms
and Conditions (provided in social media plaftorms) contest duration: 25 April 2021 to 31 May
2021 which shall be read together herein as reference (hereinafter referred to as the “T&C”).

Apart from the Prize stated herein, no other prizes or consideraton will be provided.

The Prize is not transferable and cannot be in any way changed without the express written consent
of IRDA.

In consideration of the undersigned accepting the Prize, the undersigned acknowledge and agree
that his/her submission is in line with the T&C, failure to comply with the requirements of the T&C
voids the Prize, and no substitution or other consideration will be owed to the undersigned by IRDA
and/or any party. Due to any failure by the undersigned as described herein, the Prize shall be
returned to IRDA by the undersigned, failure of which shall entitle IRDA to reserve its right on any
legal remedy available to them.

The undersigned for him/herself and all relatives, heirs, assigns, executors and administrators
release and exempt IRDA and it directors, officers, agents, or any person acting with IRDA's
authority or with its permission such as, but not limited to, any photographer, or videographer
appointed by IRDA from any and all liability, claims, actions, demands, damages which they have
now or may have at the time in the future arising from or in any way relating to the Prize.

In consideraton of the Prize awarded, the undersigned expressly acknowlegdes and agrees on the
following:

1) that all intellectual property rights of the winning entry shall belong to IRDA. IRDA shall
have the exclusive right to use, edit, modify and publish the entry submitted by the
undersigned, and the undersigned shall not claim ownership or any payment or
compensation on the in relation to the winning entry;

2) to permit IRDA on the publishing of the names of the undersigned, the use of his/her
name and likeness in any news release, adverstisement or IRDA's communications, in
any way it deems fit for any advertising, trade, promotional purpose and for any other
reason whatsoever, without any further notice to the undersigned and the undersigned
shall not claim ownership or any payment or compensation on the materials;

3) the undersigned release IRDA, and their employees, officers, directors, independent
contractors, agents, representatives, affiliates and prize sponsors from any harm,
claims, damages, losses , liabilities, injury, cost or expense whatsoever with respect to
or in connection with the Prize;

By signing below, the undersigned certify that he /she has read and fully understand the PRIZE
ACCEPTANCE FORM AND RELEASE FORM.

The undersigned further acknwoledge that this a complete release and discharge of any and all of
the undersigned claims against IRDA and their employees, officers, directors, independent
contractors, agents, representatives, affiliates and that no action will be taken by or on behalf of
the undersigned with respect to any such claim, it being understood that this release shall be
binding upon the undersigned heirs, executors and adminsitors. The undersigned further certify
that he/she is either 18 years of age or older.

PARENTS / GUARDIAN'S ACCEPTANCE:

The undersigned represengts and warrants that he/she is/are the parent or legal guardian of the
prize winner identified above who is under the age of eigteen (18 years). In consideraton of the
Prize awarded, the value and sufficiency of which are acknowledged and agreed to, consent to the
contents of the PRIZE ACCEPTANCE FORM AND RELEASE FORM on behalf of the prize winner
herein identified with full force and effect as parent and/or legal guardian, with the intent that this
release be binding for all purposes and for all time, both before and after the prize winner reaches
the age of consent, intending that the prize winner will be bound by these terms and conditions set
in herein.
