AFFIDAVIT OF REMITTANCE

I, Precy D. Cabelin, of legal age, single and Filipino citizen, do hereby state under oath that:

1. That, I am the Payroll Officer of Alcatraz Security and Investigation Agency, Inc. Part of my job
is to prepare all necessary documents and actual payment of government remittances like SSS,
ECC, HDMF, PHILHEALTH to the concerned government agency.

2. Alcatraz Security and Investigation Agency, Inc. is a duly registered Philippine Corporation and
presently the contracted Security Service Provider for the ETON PARKVIEW GREENBELT
premises located at Gamboa St., Legaspi Village, Makati City.

3. Alcatraz’s Security Contract /Arrangements with the EPG is on lump sum basis payable on a bi-
monthly basis and that said services and lump sum payment is inclusive of all services provided,
to include the cost of manpower, tools, equipment, salaries and benefits of security guards based
on the latest prevailing minimum wage law and necessary supervision;

4. All wages based on the said latest minimum wage law and government contributions of the
Security Guards stationed at EPG have been paid for by us and the corresponding contributions
remitted to the concerned government agencies;

5. As a sign of good faith; I have provided EPG photocopies of monthly SSS, PAG-IBIG, ECC,
PHILHEALTH confirmation receipts, official receipts and list of security guards detailed at
EPG; whose contributions to the above-mentioned government agencies shall be posted at a later
date:
