1. Reiteration of DEP Ed Order # 54 S. 2016


Guidelines on the Request and Transfer of Learner’s School Records
--- The LIS transfer notification shall serve as the notice of request for transfer of documents
from the originating school to the receiving school
( It replaces the traditional practice of printing the school form )
2. After 30 days of persistent follow ups made to the originating school, all pending request
shall be consolidated and submitted to the School Governance Operations Division
3. Tagging of Learners with Disabilities ( LWDs ) and recipients of School Based Feeding Program ( SBFP )
- shall be available in the LIS for end of School Year
4. LIS ( EoSY ) updating facility - available on July 5, 2021
5.. Temporary Enrolled - enrollees who failed to submit SF 9
- need to submit Affidavit of Undertaking
- Temporary Progress Card – to be issued only for progress monitoring

Answer : Magpopost sa Bulacan LIS for pending request
c. Need pa ba ng hard copy ng SF 10 kung nagpadala na ang originating school ng soft copy?
Answer : YES
d. Ano ang gagawin kung kulang ang documents ng bata dahil may balance sa private school?
Answer : Promoted siya sa LIS

e. Paano kung hindi nakoconfirm ang bata from private school dahil sa unsettled tuition fee?

END OF SCHOOL YEAR RITES FOR SCHOOL YEAR 2020-2021

1. Only Grade 6 pupils will receive Academic Excellence Award. No awards for Kindergarten
2. 2021 End of School Year Rites :

3. The short program for graduation will run in less than 2 hours to consider internet connectivity to be
consumed
4. Only the completers, parents/ guardians, teachers and school administrators will be present during the
virtual rites.
5. The schools have the discretion to broadcast live the Virtual SY rites through appropriate social media
platforms. The school shall secure the consent of the parents and learners before doing such broadcast

Answer: No
b. Maaari bang mag conduct ng meeting ang parents for graduation?
. Answer : Yes. Virtual only /or other alternative methods may be considered.
c. Paano kung walang internet access ang mga magulang para sa graduation rites?
Answer : Pwedeng recorded then ipopost live sa FB page.You may also consider other
alternative methods, considering IATF health and safety protocols
d. Ano ang magiging flow ng program sa virtual ceremony?
Answer: Ganoon pa rin ang flow ng program. Ang mga roles ng bata ay maaari pa ring gawin,
just clarify the assignment of tasks
e. Anu ano po ang maaaring i charge sa MOOE sa graduation rites?
Answer : Certificates/ Diploma, medal
f. Maari bang hiwalay ang recognition sa moving up at graduation ceremony? Maari bang sa
graduation na irecognized ang bata?
Answer: School decision with the school checking and awards committee.

ORIENTATION ON PREPARATION AND SUBMISSION OF SCHOOL FORMS

SCHOOL FORM 1- School Register

 Additional elements for learning modality


 The learning modality shall be updated whenever the learner shifts from one learning modality
 At the end of each quarter , the updated modality shall be generated through the LIS
 The age to be recorded in the form is the learners’ age by October 31, 2020

SCHOOL FORM 2- Daily Attendance

 Teachers may choose an applicable Learner Attendance conversion Tool


LACT 1 - online
LACT 2 - communication based/ applicable to all distance learning modalities
LACT 3 - output based/ applicable only to printed modular and digital modular distance
learning modalities
 Learner can be considered drop out /NLPA if he/ she and his/ her parents voluntarily withdraw from
Schooling or if he/ she incurs absences of moethan 20% of the prescribed number of class.

SCHOOL FORM 3- Books issued and Returned


 It record the distribution and retrieval of learning modules
 The title of the book shall be replaced by ref. code or title of module being distributed
 If there are existing forms being used, no need to provide

 Boxes for remarks should be filled in with the word PASSED ( all caps )
 In case of NLPA, write the reasons and date
 Indicate the honors received by the Grade 6 pupils on top of DepEd logo.

INTERIM GUIDELINES ON GIVING OF AWARDS AND RECOGNITION IN LIGHT OF THE BE-LCP


DEP ED ORDER NO. 18 S. 2021

 Suspension of some awards


They are largely from observable behavior in the classroom and various collaborative engagements
Dep Ed Order 36. S. 2016
a. Classroom Awards
- Performance Awards for Kinder, Conduct awards ( Makadiyos, Makatao,
Makabayan , Makakalikasan)
b. Grade Level Awards
- Academic Excellence Award, Leadership, Award for Outstanding Performance in
Specfic Discipline, Award for Club, Organizations
c. Specific Recognition
- Represented and/ or won in the contest

 Awards to be recognized- Academic Excellence Award for Grade 6,10, 12


 Reasons for giving Academic Excellence Awards for Grade 6, 10 and 12
a. They are highly valued by learners, their families, teachers and community
b. For scholarship grants in universities/ colleges
c. Admissions as learners transition to JHS, SHS and College

 Teachers may recognize exemplary performance through POSITIVE REINFORCEMENT during


synchronous or through the class established mechanism for giving constructive feedback to build a
levelled opportnity for all learners to excel in distance learning ( ex. Through online kamustahan )
( Grades 1-5 )
 Teachers should be more intentional about excluding from grades criteria that can perpetuate
inequities, e.g., extra-credit assignments and the likes and shares of outputs posted on social media

 It should be recognized that these things are highly dependent on a student's home
environment and resources; including them makes grading less equitable (Feldman,
2O2O).
 Learners should not fail nor receive low grades because of circumstances outside their
control.

 How to Determine the Awardees?


- an Awards Committee (AC) shall be organized by the school head
- composed of at least three (3) members from the teaching staff, guidance
counselor or designated teacher. ( odd number )
- The chairperson of the AC could be any of the teachers, department head or grade-level
chairperson. No member of the AC must be related within the second degree of
consanguinity or affinity to any of the candidates for awards

The Awards Committee shall:

a. Establish the processes of and timelines in accepting nominations and determining qualifiers for
academic excellence awards awards.
b. Communicate the processes involved in giving awards and the results of the evaluation.
c. Verify the authenticity of documents submitted (learners' portfolios that contain their written and
performance task outputs, class records, report cards, and other school forms).
d. Ensure accuracy in the computation of grades of the candidates.
e. Recommend to the school head the result of evaluation and deliberation for approval.
f. Recommend to the school head the resolution on any related issue that may arise from the results of
the awards.
 The AC shall use the report cards and permanent records supported by the learners’ portfolio as the
main reference for determining the awards.
The report of the AC shall be signed by all the members of the committee and certified by the school
head. The school head shall approve the final list of awardees upon the recommendation of the
committee.

 Awardees in Grades 6 shall receive medals from the school, bearing the official seal of the
Department.

Program of activities which may be revised or adapted by the schools:

e. Inspirational Message
f Presentation of the School Officials, Teachers, Guests, Parents, and Awardees

 The names of awardees shall be done in alphabetical order
 School shall release an official announcement of the awardees by posting the list in
the school bulletin board and shall be communicated through the agreed communication platform
 certificate and medal shall be given to awardees or parents and receipt shall be acknowledged
 The school head may schedule a date when the parent and awardee may drop by in the
school to receive the award and recognition.
 Cases of protest shall be filed to the office of school head within three (3) working days from the
receipt of the announcement of the awards and shall be decided on by the school head considering
the recommendations of the AC within three (3) working days from filing.
