Your trainer will give you the Assessment A - Risk Assessment template.
Save the template locally to your computer and complete the risk assessment template.
Place the risks you identified on the risk assessment matrix and prioritise at least three of these in
order from most important to least important to address.
Save and submit your template to your assessor when completed.
Q2: What steps are involved in a risk assessment according to your organisation’s
procedures?
=The steps which are involved in a risk assessment according to our organization’s
procedures are;

=The new preventive measures that we could implemented for risk assessment are;

=Work health and safety act 2011 is the industry standard legislation that the new
procedures comply with.
Q4: What issues did you identify during the consultation process and what action did you take
to resolve the issues?
=The issues we identify during the consultation process is it is difficult to consult during
the busy time as we cannot neglect our service to the customer so we should organize the
extra staffs in the roster.

your consultation activities in Task 4.
=Suggested management to roster extra staffs on duty so that we can train the staffs
about the related topic and along with we can carry out our business activities.

=Yes, we can put extra staffs on the roster so that training can be done and workplace
activities also can be carried out without any issues.
Q7: Were the consultative processes effective in identifying issues and communicating with
staff? What improvements would you make for the future?
=Yes, the consultative processes were effective in identifying issues and communicating
with the staffs but some of the staffs did not attend the program so from next time we will
make sure all the staffs attend the program.

=No, there is no any another area of WHS that we are concerned about after complying
our monitoring activities.

health, safety and security of personnel?
=We can monitor the day to day effectiveness of WHS practices by;
I. Observe them on daily basis.

II. Consult with manager and staffs.
Q2: How long does it take to arrange and complete the training from the time the training need
was identified?
=It takes around 2 weeks to arrange and complete the training from the time the training
need was identified.

=Yes, the training improved the employees work practices which leads to less accidents in
the workplace.

=Work health and safety act 2011 is the legislation we need to refer to when implementing
and monitoring the WHS procedures.

=The following data are required;

Q4: How often should WHS documents be referred to, to check the effectiveness of WHS
procedures?
=WHS documents should be referred to check the effectiveness of WHS procedures on
regular basis like weekly.
Q5: You notice that some team members are not stacking the shelves correctly and
according to work safety procedures in the storeroom. You have just seen some heavy
items stored on top. What is your response? When would you deal with this issue?
=In such situation we should Immediately deal with issue and if necessary, re-training
program should be organized.
Q6: You are looking through the WHS incident reports and notice that many of the forms are
difficult to read due to the illegibility of the handwriting. It is really important for anyone to
be able to read these documents in order to analyse the information in them. How and
when would you deal with this issue?
Q7: What action can you take to minimise the use of printed materials and reduce waste when
organising WHS documentation?
=To minimize the use of printed materials and to reduce the waste we should records the
documents systematically and communicate via emails or messages to transfer the
information.

=The scheduled hazard identification activities are
