DATE: FEBRUARY 23, 2020

The matter of your absent has been brought to our attention. In particular, it appears that you failed to report for work
on February 16, 2020 onwards.

Records show that your absent was not authorized under an approved leave application. In this regard, please be
informed that unauthorized absent or absences without leave constitutes gross and habitual neglect of duties and may
affect the final output of the project target date accomplishment, this matter is a ground for termination under the Labor
Code. On record, this is a total of 3 days of unauthorized absences.

In view of the forgoing, you are hereby required to explain within five (5) days from receipt of this notice, stating why
the Company should not terminate your employment and a letter that you will not doing it again.

Furthermore, you are invited to a conference on November 28, 2019 at Unit 110 Union Square Condominium, Barangay
Socorro, 15th Avenue, Cubao, Quezon City where you shall be given the opportunity to express your side on the matter.
You may be accompanied by your own counsel at the said conference if you wish to be assisted by one.

Your failure to submit your written explanation within the period provided shall be construed as a waiver of your right to
be heard and the Company shall decide the matter on the basis of all records and evidence available to it.

Your compliance is strongly encouraged.
