NCP For Postpartum

A postpartum woman presented with continued vaginal bleeding after delivery. The nurse assessed the woman had ineffective tissue perfusion related to vaginal bleeding. The short term goal was for the patient to demonstrate adequate perfusion after 8 hours of nursing intervention including vital sign monitoring, blood gas monitoring, oxygen therapy, and measuring blood loss. The long term goal was for the patient's vital signs and blood gases to be within normal limits after 1 week. Nursing interventions focused on monitoring, oxygen therapy, uterine massage, and comfort measures to improve tissue perfusion and control bleeding.
