For Students/Employees/Households affected by the Tropical Storm Sendong floods during December 16
and 17, 2011.
We are conducting this survey so that we can better understand how you, your household and your community
were affected during Tropical Storm Sendong floods during December 16 and 17.
We realize that this is quite a long survey questionnaire and we hope that you will respond to all of the
questions. However, you do not have to complete every question. We appreciate the time that you are taking to
provide us with this valuable feedback but please feel free to skip over any questions that you are unable to, or
do not wish to, answer.
The information you provide will help us assess how to improve storm and flood warning services in our
region. It will also assist us in our work with our partner agencies to provide more efficient flood mitigation,
particularly in terms of community preparation and response.
Please follow the instructions when answering each question. Only your responses are recorded. We do not
know your identity. You are welcome to choose to provide your details and any additional feedback through the
spaces provided below.
